ObjectiveBased on the China Health and Elderly Follow-up Survey, this paper studied the influencing factors of cognitive dysfunction in the elderly population in China. The aim is to understand the status of cognitive dysfunction in the elderly in China, and to analyze the influencing factors and developmental effects of cognitive dysfunction in the elderly. The establishment of the elderly cognitive dysfunction discrimination model and the search for new ideas of social medical resources allocation are convenient for the elderly with cognitive dysfunction and their families to carry out targeted education and health guidance, effectively avoid cognitive dysfunction in the elderly population, and provide the basis for improving the quality of life of the elderly and enjoying their old age.MethodsThis study uses data from China Health and Aging Tracking Survey of Peking University to conduct research. The literature research method is used to study and sort out the existing research at home and abroad.Statistical description was used to describe the demographic characteristics, health status, life behavior, chronic disease, sleep and depression of the elderly population.Linear regression model was used to explore the relationship between various influencing factors and cognitive dysfunction, and the influencing factors of cognitive dysfunction in the elderly were analyzed. The Lasso regression model was used to construct the cognitive dysfunction discriminant model in the elderly population.ResultsThis study takes the elderly population over 60 years old as the research object, with a total of 3256 samples, among which there are 2130 elderly men and 1126 elderly women, ranging in age from 60 to 92 years old.The results showed as follows :(1)The average MMSE score of the elderly was(23.874 ± 4.247).The prevalence of cognitive dysfunction was 37.4%(95%CI(0.357,0.390).(2)the single factor showed that demographic characteristics(age, education level, residence type, region distribution, marital status),lifestyle(drinking),social activities, chronic disease(blood pressure, liver disease, heart disease, rheumatism),sleep, depression and disability, daily life ability difference is statistically significant;Logistic regression analysis showed that the influencing factors of cognitive dysfunction in the elderly included demographic characteristics(age, education level, residence type, regional distribution, marital status),life behavior(social activities),chronic disease(liver disease, heart disease),depression, daily living ability and sleep;(4)The ROC curve evaluation results of the cognitive dysfunction discriminant model showed that the optimal threshold point was 0.337,the sensitivity was 0.494,the specificity was 0.825,the positive predictive value was 0.628,the negative predictive value was 0.732,and the AUC value was 0.705.ConclusionsThe overall cognitive function of the elderly population in China is relatively low, among which the proportion of patients with cognitive dysfunction is 37.4%,95%CI(0.357,0.390).The study found that living in rural areas was the most likely factor to increase the likelihood of cognitive dysfunction in the elderly, followed by depression.Other factors included advanced age, single marital status, and regional distribution. The factors that can reduce the possibility of cognitive dysfunction in the elderly are high education level, frequent participation in social activities and normal ability of daily living. The risk of cognitive dysfunction in patients with chronic diseases is actually reduced, which needs further study. The identification model of cognitive dysfunction based on Lasso regression is relatively low in sensitivity, but it has high specificity, and can be used to screen out non-patients more accurately, which has certain practical value.
