The Relationship Between Platelet Membrane Glycoprotein Specific Antibody And Bleeding Score And The Efficacy Of HD-DXM In Patients With ITP

Objective:In the etiology of ITP,the formation of platelet membrane glycoprotein(GP)-specific antibodies mediated by B cells plays a major role in the pathogenesis.This article aims to explore the relationship between the expression of antiplatelet GP IIb/IIIa and GP Ib/IX antibodies,bleeding scores and the efficacy of HD-DXM in patients with ITP.Methods:A collection of 100 patients with ITP admitted to the Hematology Center of the First Affiliated Hospital of Xinjiang Medical University,use the modified MAIPA method to detect anti-GP IIb/IIIa and GP Ib/IX antibodies before HD-DXM(40mg/d×4d)treatment,analyze antibody expression,bleeding score and HD-The relationship between the efficacy of DXM.Results:(1)Comparison of general data between the effective and ineffective groups of ITP patients showed statistical differences in ethnicity(P<0.001)and bleeding score(P=0.033);(2)Han group:the effective rates of treatment for patients with positive anti-GP IIb/IIIa antibody,positive anti-GP Ib/IX antibody,positive for both antibodies,and antibody negative were 80.0%,75.0%,50.0%and 96.4%,respectively,the comparison of antibody expression types and efficacy among the four groups is statistically significant(?~2=12.075,P=0.007);Uyghur group:the effective rates of treatment for patients with anti-GP IIb/IIIa antibody positive,anti-GP Ib/IX antibody positive,double antibody positive and antibody negative were 50.0%,33.3%,12.5%,70.6%,the comparison of antibody expression types and efficacy among the four groups is statistically significant(?~2=9.349,P=0.025);(3)Logistic regression model:Compared with Han nationality(P<0.001),single anti-GP Ib/IX antibody positive(P=0.012),double antibody positive(P<0.001)and double antibody negative are negatively correlated with the efficacy of HD-DXM in Uyghur patients.(4)The area under the ROC curve(AUC),calibration curve and DCA curve were used to verify the Nomogram model,suggesting that it is feasible to analyze the efficacy of HD-DXM in patients with ITP.Conclusion:After HD-DXM treatment in ITP patients,there are differences in ethnicity and bleeding scores between the effective group and the ineffective group.Uyghur patients and patients with positive expression of anti-GP Ib/IX antibodies have poor therapeutic effects.
