Factors Influencing The Participation Of Patients With COPD In Pulmonary Rehabilitation

Abstract/Summary:
Objective:(1)Investigate the cognition of chronic obstructive pulmonary disease(COPD)patients on disease and pulmonary rehabilitation(PR).(2)Clarify the factors affecting the completion of pulmonary rehabilitation program in patients with chronic obstructive pulmonary disease.Methods:(1)In this study,recruited 165 patients with chronic obstructive pulmonary disease who were admitted to the Respiratory and Critical Care Medicine Center of the People’s Hospital of Xinjiang Uygur Autonomous Region from January to November 2020,completed the COPD cognition questionnaire,and describing the results of the questionnaire by frequency and composition ratio.Grouping the patients according to different basic information,and comparing the questionnaire scores of different groups by two independent sample t-tests and single factor variance analysis,and using multiple linear regression analysis to analyze statistically significant variables.(2)Carried out education on diseases and lung rehabilitation to the patients who have completed the questionnaire,and drawn up a pulmonary rehabilitation plan,made a follow-up call every 4 weeks to understand the implementation of the pulmonary rehabilitation plan.After 12weeks,completed the m MRC and cat scores.If the rehabilitation plan was not completed,asking the reasons for the incompleteness,and divided the patients into the complete group and the incomplete group according to the completion of the pulmonary rehabilitation.Comparing the general information and questionnaires,scales,pulmonary function,the blood gas analysis results of the patients between the groups,and using the binary logistic regression analysis on the indicators with differences between groups.Results:(1)In this study,handing out 165 questionnaires,162were effectively recovered,with an effective recovery rate of 98%.Among all subjects,the overall awareness of disease was 55.5%,and the overall awareness of pulmonary rehabilitation was 28.3%.Patients with blow average awareness of disease and pulmonary rehabilitation accounted for 19.1%(n=31)and72.8%(n=118).The Single factor analysis showed that if the patient’s age,address and education level,occupation are different,the questionnaire results will be different,and the difference was statistically significant(P<0.05).The results of multiple linear regression analysis showed that age and education level are negatively correlated with the questionnaire score,which is an influencing factor of the questionnaire score.(2)A total of 159 patients were followed up for 12 weeks,of which 43.4%(n=69)patients completed the program,while 56.6%(n=90)patients did not complete.The results of single factor analysis showed that there were statistically significant the differences between the two groups in variables such as ethnicity,address,BMI,monthly household income per capita,FEV1,FEV1/FVC,Pa O2,Pa CO2,m MRC,cat score,HAMD,HAMA,COPD cognitive questionnaire scores(P<0.05).The results of binary logistic regression analysis showed that the address,per capita monthly income,FEV1,FEV1/FVC,m MRC,cat score,HAMD,HAMA,COPD cognitive questionnaire scores have an impact on the completion of the patient’s pulmonary rehabilitation plan.Among them,The higher the variable values of m MRC,cat score,HAMD and HAMA,the worse the compliance of pulmonary rehabilitation.The reasons why patients with COPD cannot complete the pulmonary rehabilitation program include:lack of motivation,fear of damaging health,acute exacerbation of COPD,hospitalization(except hospitalization due to exacerbation of COPD),working busy,family responsibilities,etc.Conclusion:(1)Medical staff need to strengthen the education of disease and pulmonary rehabilitation for all patients with chronic obstructive pulmonary disease,especially those who are older,low level of education.(2)Evaluate the compliance of patients with pulmonary rehabilitation through the influencing factors of pulmonary rehabilitation and intervene the patients with poor compliance to improve the completion rate of pulmonary rehabilitation programs.
