Purpose:The imaging factors affecting the formation of intracranial aneurysms with daughter blebs were analyzed and obtained by statistical analysis,so as to provide a reference imaging basis for the detection of intracranial aneurysms with daughter blebs.Materials and methods:A retrospective analysis was made on 344 patients with intracranial aneurysms confirmed by DSA or operation and undergoing CTA examination in China-Japan Friendship Hospital of Jilin University from January 2017 to August 2020.A total of 396 cases were diagnosed as intracranial aneurysms,including 119 cases with daughter blebs and 277cases without daughter blebs.Measure and collect the clinical information related to the patients:1.Basic clinical data:age,sex,smoking history,diabetes history,hypertension history,systolic blood pressure(SBP),diastolic blood pressure(DBP),classification of hypertension.2.Imaging data:responsible aneurysm location,size,neck,height,maximum diameter,aspect ratio,aspect ratio,incident angle,width/neck.Z test(non-normal distribution numerical variable and ordered grade classification variable)and chi-square test(independent classification variable)were used to analyze the above factors,and bivariate Logistic regression analysis was carried out to find the independent risk factors for the formation of intracranial aneurysms with daughter blebs.The ROC curve was drawn and the critical value was calculated.Results:Univariate analysis showed that the age of the patient(Z=-1.623,P=0.105),gender(χ2=1.077,P=0.299),smoking history(χ2=0.023,P=0.879),diabetes(χ2=0.040,P=0.842),hypertension(χ2=1.380,P=0.240),systolic blood pressure(Z=-0.054,P=0.957),systolic blood pressure(Z=-0.263,P=0.792),hypertension grade(Z=-0.072,P=0.942),aspect ratio of responsible aneurysm(Z=-0.452,P=0.652),diameter/width(Z=-0.875,P=0.381),bottleneck(Z=-1.073,P=0.283)were no significant difference.There were significant differences in neck(Z=-5.662,P=1.498×10-9),height(Z=-5.380,P=7.443×10-8),the maximum diameter(Z=-4.980,P=6.375×10-7),incidence angle(Z=-2.130,P=0.033),size(Z=-5.548,P=2.892×10-8)and location of responsible aneurysm(χ2=18.499.P=0.002).The results of multivariate Logistic analysis showed that the neck of responsible aneurysm(OR=1.695,P=3×10-6),the maximum diameter of responsible aneurysm(OR=1.136,P=0.01)and the location of responsible aneurysm(P=0.029)were independent risk factors.The area under curve of the maximum diameter of responsible aneurysm was 0.658,the critical value was 4.45,and the area under curve of the neck was 0.679,and the critical value was 3.85.Conclusion:The neck and the maximum diameter and the location of responsible aneurysm are independent risk factors for IA to form daughter blebs.The critical value of the neck of the responsible aneurysm is 3.85mm,and the critical value of the largest diameter is 4.45mm.
