Objective:Through a retrospective study,analyze the data of 1,159 patients with CHD from February 2016 to October 2019 in the Department of Cardiovascular Medicine of our hospital,to summarize the characteristics of clinical data and the operative effect and survival condition of interventional therapy,then provide theoretical basis for the choice of treatment methods for various subtypes of CHD patients.Methods:Collect inpatient medical records and follow-up data of CHD patients in the Department of Cardiovascular Medicine of our hospital from February 2016 to October2019 were collected.Through retrospective study,the epidemiological characteristics of CHD and the efficacy of interventional treatment were analyzed and summarized.The collection includes:1)basic information: Sex,name,age,nationality,residential address;2)weight,BMI,heart rate,blood pressure,diagnosis,genetics and family history,complications,pulmonary artery pressure,left ventricular echocardiography,Arrhythmia,etc.3)follow-up: The length of the patient’s stay,the type of interventional treatment,the method of anesthesia,preoperative and postoperative heart color ultrasound,and whether there were complications during the perioperative period.Such as: Postoperative Shunt,pericardial tamponade or effusion,Arrhythmia,puncture site Hematoma,postoperative hemolysis,postoperative Thrombocytopenia,postoperative valve insufficiency.Results:From February 2016 to October 2019,a total of 1159 patients with CHD were admitted to our department of Cardiovascular Medicine,including 412(35.5%)males and747(64.5%)females,with an average age of 18.24 years,a total of 375 cases(32.3%)were admitted in 2018 and 287 cases(24.7%)in 2019.The subtypes of CHD in hospitalized CHD patients included: ASD in 768 cases(66.2%),VSD in 87 cases(7.5%),PDA in 174 cases(15.0%),PFO in 106 cases(9.4%),PS in 7 cases(0.6%),complex lesions such as ASD + VSD,ASD + PDA,VSD + PDA,PDA + PS in 12 cases(1.3%),there were 5 cases(0.4%)with other diseases such as coronary artery leak,shunt after ASD and VSD repair,and shunt after PDA.Among 1152 cases,766 cases(66.5%)underwent ASD closure,104 cases(9.0%)underwent PFO closure,86 cases(7.5%)underwent VSD closure,173 cases(15.0%)underwent PDA closure,7 cases(0.6%)underwent PS balloon dilation,and 12 cases(1.0%)underwent combined closure,other operations were performed in 4 cases(0.3%).A total of 359 cases of arrhythmia were found in postoperative complications,including 14 cases of atrial fibrillation,24 cases of different degrees of atrioventricular block,33 cases of complete right bundle branch block,3 cases of pre-excitation syndrome,177 cases of arrhythmia in the junction area,and 108 cases of other atrial and ventricular arrhythmias.The occluder dropped out in 1 case and pericardial effusion in 2cases.There were 1 case of hemolysis and 1 case of thrombocytopenia,1 case of puncture point hematoma,and a total of 6 cases of infection and fever.766 cases of ASD were treated by interventional therapy.The average preoperative pulmonary artery pressure was 30.52±10.53 mm Hg and the average hospitalization time was 5.29 0.98 days in the 1-6 year-old group.The mean pulmonary arterial pressure was33.79±12.68 mm Hg and the mean hospital stay was 5.57±1.75 days in the 7-to 17-yearold group.The mean preoperative pulmonary arterial pressure was 41.00±15.01 mm Hg and the mean hospital stay was 6.9±2.76 days in the ≥18-year-old group.In 173 cases of PDA intervention therapy,the average pulmonary artery pressure before operation in the1-6 year-old group was 27.54±10.39 mm Hg,and the average hospital stay was 5.08 ±0.98 days.The mean preoperative arterial pressure in the 7-17 year-old group was32.15±15.00 mm Hg,and the average hospitalization duration was 5.27±1.36 days.The mean preoperative pulmonary arterial pressure in the ≥18-year-old group was45.38±25.17 mm Hg,and the mean hospital stay was 6.56±3.01 days.In the ASD group,the values of preoperative echocardiography were RA3.48±0.69,RV2.33±0.68,and LVD4.01±0.63;Postoperative echocardiography values were RA3.19±0.58,RV2.07±0.54,and LVD4.18±0.64.In the PFO group,the preoperative echocardiography values were LA2.95±0.60,RA3.31±0.61,RV1.80±0.36,LVD4.37±0.56,and EF(%)0.64±0.03.Postoperative echocardiography values were LA2.44±0.52,RA2.89±0.60,RV1.60±0.35,LVD4.16±0.52,and EF(%)0.67±0.59.In the PDA group,the preoperative echocardiography values were RV1.51±2.6 and LVD4.22±0.90.Postoperative echocardiography values were RV1.71±3.9 and LVD3.84±0.80.In the VSD group,the preoperative echocardiography values were RA2.89±0.68,RV1.77±0.72,LVD4.20±0.82,and EF(%)0.71±0.05.Postoperative echocardiography values were RA2.53±0.57,RV1.58±0.66,LVD3.69±0.97,and EF(%)0.62± 0.07.Conclusions:1.The number of interventional cases of CHD increased year by year from February 2016 to October 2019.There were more females than males in the hospitalized patients,and the difference was statistically significant.2.ASD and PDA are the most common disease types in the intervention of CHD.3.Arrhythmia is the most common complication of CHD intervention.4.Pulmonary artery pressure in ASD and PDA patients increased gradually with age,and the hospitalization time of patients was prolonged.5.Patients with ASD,PFO,PDA and VSD had decreased heart size and improved structure after intervention.
