Background: Depression is a common psychiatric disease, and it is very easy to relapse, and the course of the disease is mostly chronic. There are still a large number of patients with antidepressants whose efficacy is not good, and there is a lack of objective indicators for the evaluation of antidepressant efficacy and long-term prediction. The changes in emotions can have corresponding changes on the EEG,and the frontal lobes of the left and right cerebral hemispheres react differently to changes in emotions.Quantitative evaluation related indicators based on EEG can be used to evaluate whether antidepressants can take effect early and predict long-term efficacy. Objective:Through the diagnosis and treatment of antidepressants for patients with major depression, the Alpha wave energy of the left and right prefrontal lobes of the brain is collected in the resting state of the patient’s EEG,and the asymmetry value of the Alpha wave energy of the prefrontal lobe of the brain is calculated, and the depression-related scale score is used as an evaluation method. The changes in the asymmetry value of the Alpha wave energy of the prefrontal lobe of the brain and the depression-related scale scores were compared before and after drug treatment. To explore whether there is a difference in the energy asymmetry of the prefrontal lobe of the brain between patients with early onset of treatment and those with no effect, and the correlation with the efficacy of antidepressants, and to evaluate the prefrontal lobe of the Alpha wave in the brain to predict whether current drugs can be early The sensitivity and specificity of onset and prediction of long-term curative effect are compared with the whole brain electrophysiological indicators, whether the left and right cerebral hemisphere physiological indicators predict curative effect more accurately. At the same time,it provides a certain reference basis for the use of EEG neuro-electrophysiological indicators to predict whether current drugs can take effect early and evaluate the prognosis in clinical applications,so as to adjust the drug treatment plan in time.Method: The enrolled patients were randomly given sertraline hydrochloride tablets or paroxetine hydrochloride tablets. The patients were measured for electroencephalogram and neurophysiological indicators before treatment and 1 week after treatment. At the time of enrollment, week 1,and 2 Relevant depression scale scores were performed on week,4th,and 6th week to compare whether there is a difference in the asymmetry of the prefrontal Alpha wave energy between patients who started and ineffective within 2weeks of drug treatment. After 1 week of treatment, whether there is a correlation between the asymmetry change in Alpha wave energy of the prefrontal lobe of the brain and the reduction rate of the Hamilton scale in the second week. Can the asymmetry of the energy asymmetry of the prefrontal lobe of the brain after 1 week of treatment be used to predict whether the current drug can take effect early? Whether the asymmetry value of the Alpha wave energy of the frontal lobe of the brain in the first week is correlated with the reduction rate of the Hamilton Depression Scale in the sixth week, and whether it can predict the efficacy of antidepressants. To compare whether the two drugs have any difference in the degree of change of the energy asymmetry of the prefrontal lobe Alpha wave, the statistical software uses the SPSS25.0 statistical analysis software package.Result:A total of 60 patients were enrolled in this study, including 30 patients in the sertraline hydrochloride tablets treatment group and 30 patients in the paroxetine hydrochloride tablets treatment group.7 cases fell off during the follow-up.Among the53 patients who finally completed the follow-up,the average age was 41.72±8.63 years old. There were 18 males and 35 females;39 were married and 14 were unmarried;7had a positive family history and 46 had no family history;medication status: 27 cases took sertraline tablets and 26 cases took paroxetine tablets. After 1 week of regular drug treatment, the difference between the two groups and the overall comparison of the energy asymmetry of the prefrontal lobe of the brain before and after 1 week was statistically significant(P<0.05).There was no significant difference in alpha wave energy asymmetry between the two groups before and 1 week after treatment(P >0.05).After 2 weeks of antidepressant treatment, there was no statistically significant difference in gender, age, asymmetry of Alpha wave energy in the prefrontal lobe of the brain before and 1 week after the overall comparison of the early drug onset within 2weeks of treatment(P>0.05),after 1 week of treatment, the difference in the energy asymmetry of the prefrontal lobe of the brain was statistically significant(P<0.05).After taking the medicine regularly for 2 weeks, the changes in the energy asymmetry of the Alpha wave energy in the prefrontal lobe of the brain were positively correlated with the reduction rate of the Hamilton Depression Scale in the second week(R=0.485,P<0.05).After taking the medicine regularly for 6 weeks, the energy asymmetry of the prefrontal lobe of the brain at the first week was positively correlated with the reduction rate of the Hamilton Depression Scale at the sixth week(R=0.543,P<0.05).After 1week of treatment, the asymmetry change value of the Alpha wave energy in the prefrontal lobe of the brain is used to judge whether the current drug can take effect early. The sensitivity is 75%,and the specificity is 82.9%.After 6 weeks of treatment, the asymmetry value of the Alpha wave energy in the prefrontal lobe of the brain in the first week was used to determine whether the drug treatment was effective in the sixth week. The sensitivity was 80% and the specificity was 65.2%.Conclusion: After one week of treatment, the value of alpha wave asymmetry in prefrontal cortex changed, but there was no difference between different drugs. The value of alpha wave asymmetry and its change after one week of treatment can predict the long-term efficacy and early onset of drugs.Compared with the whole brain electrophysiological indexes, the sensitivity of left and right cerebral hemisphere physiological indexes in predicting long-term curative effect may be improved, but the specificity is decreased.
