Clinical Significance Of Triglyceride Glucose Index In Patients With Coronary Heart Disease

Abstract/Summary:
Objective To explore the clinical significance of triglyceride glucose index(TyG index)in patients with CHD.Methods Three hundred CHD patients were collected and divided into T2DM group and non-T2DM group according to the history of type 2 diabetes mellitus(T2DM).The TyG index of the patients was calculated,and its relationship with the blood glucose control level,risk factors and the severity and stability of CHD was analyzed.TyG index=Ln[fasting triglyceride(mg/dL)× fasting glucose(mg/dL)/2].Results TyG index of T2DM group was higher than that of non-T2DM group(P<0.001).The T2DM group was further divided into good glycemic control subgroup(HbAlc<7.0%)and poor glycemic control subgroup(HbAlc≥7.0%).TyG index of poor glycemic control subgroup was higher than that of good glycemic control subgroup(P<0.05).Spearman correlation analysis showed that TyG index was positively correlated with the HbAlc(r=0.425,P<0.001).In non-T2DM patients,the TyG index of overweight group was higher than that of non-overweight group(P<0.05).While in T2DM patients,there was no significant difference of TyG index between overweight group and non-overweight group.TyG index was significantly positively correlated with BMI,total cholesterol,low density lipoprotein cholesterol,HbA1c and uric acid(P<0.05),and negatively correlated with high density lipoprotein cholesterol(P<0.05),regardless of whether CHD patients were combined with type 2 diabetes mellitus(P<0.05).With or without T2DM,there was no significant correlation between TyG index and the number of coronary lesions and Gensini score.However,TyG index of acute coronary syndrome(ACS)group was higher than that of stable angina pectoris(SAP)group(P<0.05).The AUC of ACS predicted by TyG index was 0.679.Conclusion1.TyG index was associated with the presence or absence of T2DM in patients with CHD and can be used as an indicator of blood glucose control in patients with CHD and T2DM.2.In CHD patients without T2DM,TyG index was associated with overweight.3.The TyG index was significantly higher in patients with ACS than in patients with SAP,and could be used as a predictor of clinical instability events in CHD.
