Effect Of Xuezhikang Combined With Ezetimibe On Visceral Fat Area In Patients Of Hyperlipemia

Objective:Existing evidence indicates that the basic pathophysiological mechanism of lipid metabolism disorders is visceral fat accumulation and dysfunction.Xuezhikang is a clinically commonly used Chinese Patent drug.This study is expected to confirm that its Xuezhikang combined with ezetamibe could reduce visceral fat area accumulating in patients of hyperlipidemia,and improving related metabolic disorders.Methods:According to inclusion criteria and exclusion criteria set in this study,the objects of this study were patients diagnosed as hyperlipidemia(phlegm turbidity and blood stasis type),treated in the department of cardiology,the institute of hypertension and the ward of the department of cardiology of the affiliated hospital of Nanjing University of Traditional Chinese Medicine(Affiliated Hospital of Nanjing University of Traditional Chinese Medicine)from April 2020 to March 2021.A total of 95 hyperlipidemia patients with phlegm turbidity and blood stasis type were enrolled,to analyze the correlation between visceral fat area and related indexes、TCM syndromes.A total of 41 patients joined the clinical study of effect of Xuezhikang combined with ezetimibe on visceral fat area in patients of hyperlipemia,randomly divided into experimental group and control group,the experimental group containing 18 patients,the control group containing 23 patients.The experimental group was applied Xuezhikang combined with ezetimibe,the control group was applied combined atorvastatin with ezetimibe,for 4 weeks.On all its object of study of general information to collection and sorting,such as gender,age,etc.All objects were measured fat area using Omron Dualscan HDS-2000.The patients included in the study of anthropometry index measurement,such as height(cm)and weight(kg),BMI(kg/m2),waist circumference(cm),hip circumference(cm),and the related statistical collection of biochemical indexes:blood lipid metabolism index:total cholesterol(TC),triglyceride(TG),high-density lipoprotein cholesterol(HDL-C),low density lipoprotein cholesterol(LDL-C),etc.Safety indexes:alanine transferase(ALT),aspartic transferase(AST),creatine kinase(CK),creatine kinase isoenzyme(CK-MB),y-glutamyl transferase(y-GT),serum creatinine(SCR),urea nitrogen(BUN),uric acid(UA),etc.,and recorded the changes of TCM syndrome scores were.Results:1.Pearson correlation analysis showed that visceral fat was positively correlated with body weight,BMI,waist circumference,hip circumference,and TG(r=0.699,r=0.666,r=0.682,r=0.533,r=0.403,P<0.01),and negatively correlated with HDL-C(r=-0.394,P<0.01).Visceral fat was negatively correlated with TCM syndromes total score,primary TCM syndromes score,secondary TCM syndromes score(r=-0.129,r=-0.167,-r=-0.029,P<0.01),negatively correlated with chest tightness syndromes score(r=-0.226,P<0.05).2.There were statistically significant differences in body weight and BMI between the experimental groups before and after treatment(P<0.05).In the control group,the difference between TC before and after treatment was statistically significant(P<0.05),and the difference between TG and LDL-C before and after treatment was statistically significant(P<0.01).Before and after treatment,there was no significant difference in visceral fat area and abdominal subcutaneous fat between the experimental group and the control group(P>0.05).3.After treatment,the TCM syndromes total score and primary TCM syndromes score in the experimental group and the control group were significantly improved(P<0.01),while the changes in secondary TCM syndromes score were not statistically significant(P>0.05).Before and after treatment,the TCM syndromes total score、primary TCM syndromes score of the experimental group and secondary TCM syndromes score were lower than those of the control group,but the differences were not statistically significant(P>0.05).Before and after treatment,the scores of all syndromes in the experimental group were improved.The improvement of chest tightness syndrome and vomiting sputum syndrome of primary TCM syndromes was statistically significant(P<0.01),and the improvement of palpitation syndrome of secondary TCM syndromes was statistically significant(P<0.05).In the control group,the scores of all TCM symptoms score were improved after treatment,but the difference was not statistically significant(P<0.05).The total effective rate of experimental group was 77%,which was better than that of control group.Conclusion:Visceral fat was positively correlated with body weight,BMI,waist circumference,hip circumference and TG,and negatively correlated with HDL-C;Visceral fat was positively correlated with TCM syndromes total score,primary TCM syndromes score,secondary TCM syndromes score and hest tightness syndromes score.The comparison of XuezhiKang combined with ezetimibe and Atorvastatin combined with ezetimibe may reduce the tendency of visceral fat in patients with hyperlipidemia and in line with phlegm turbidity and blood stasis type,reduce the blood lipid of patients and improve the traditional Chinese syndromes,which is worthy of further study.
