Objective:Gastrointestinal dysfunction is a common and complex complication in patients with severe diseases and surgical patients,and the prevalence rate in intensive care unit patients is as high as 60%.Gastrointestinal dysfunction seriously threatens the prognosis of critically ill patients,prolongs the hospital stay,and has a significant impact on both clinical and economic conditions.Mild gastrointestinal dysfunction is an acute,short-term,self-limiting disease,manifested as nausea,abdominal distension,impaired gastrointestinal motility,eating intolerance,and intestinal obstruction.Severe gastrointestinal dysfunction is a common comorbidity of severe diseases,including non-occlusive mesenteric ischemia,intestinal infarction,gastrointestinal perforation and peritonitis.The occurrence of gastrointestinal dysfunction may be related to intestinal ischemia-reperfusion injury,especially during the period of hypotension with vasopressor drugs,the mortality rate can increase by 8 times.Gastrointestinal dysfunction plays an important role in the pathogenesis of MODS and MOF.Effective prevention and treatment of gastrointestinal dysfunction is the key to preventing the occurrence and development of multiple organ dysfunction in critically ill patients and reducing the mortality rate.Traditional Chinese medicine has a lot of literature on the treatment of critically ill gastrointestinal dysfunction.This research is carried out to explore the advantages of traditional Chinese medicine in order to improve clinical efficacy.Method:This study uses prospective,randomized,and controlled research methods.A total of 64 critically ill patients who were admitted to the EICU of the Emergency and Intensive Medicine Department of Jiangsu Integrated Traditional Chinese and Western Medicine Hospital and the emergency department of the Southern District of Jiangsu Integrated Traditional Chinese and Western Medicine Hospital from September 2018 to March 2021 were randomly divided into treatment group and control group.32 cases.Routine treatment should actively treat the primary disease,maintain the gastrointestinal mucosal barrier,give gastrointestinal motility drugs,provide early nutritional support,and gastrointestinal decompression.On the basis of conventional treatment,the treatment group received electroacupuncture from the day when the enteral nutrition was opened,and the treatment days were from the start of the enteral nutrition until the target enteral nutrition reached the target.Observe the patient’s baseline data,the time required for enteral nutrition to reach the standard,the duration of the ICU,the total length of hospitalization,the length of mechanical ventilation,the survival during the hospitalization,and the gastrointestinal dysfunction on the 1,3,and 7 days after admission Score,TCM syndrome score,nutrition-related indicators,AGI grading after treatment.To discover and evaluate the acupuncture-related adverse events during the adjuvant electroacupuncture treatment and the safety of electroacupuncture treatment.Result:There was no statistical difference between the two groups in gender,age,past medical history,primary disease,APACHE II score,and AGI classification(p>0.05).After electroacupuncture,the time to reach the standard of enteral nutrition in the treatment group(3.91 ± 0.73 days)was shorter than that in the control group(4.06±0.67 days),but the difference was not statistically significant(p>0.05).After electroacupuncture,the gastrointestinal dysfunction score,AGI grade,and syndrome score of the treatment group were improved compared with the control group,and the difference was statistically significant(p<0.05).In the observation of nutrition-related indicators,it was found that after electroacupuncture treatment,hemoglobin(126.84± 16.31g/L),serum albumin(32.53±2.98g/L)and serum prealbumin(0.21±0.06)g/L)level was higher than that of the control group,and the difference between the two groups was statistically significant(P<0.05).At the same time,the time of invasive mechanical ventilation in the treatment group(4.63±0.71 days)was shorter than that of the control group(6.31 ± 3.86 days),and the difference was statistically significant(p<0.05).There was no significant difference between the two groups in ICU stay,hospital stay,and mortality during hospitalization(p>0.05).Conclusion:Acupuncture on the bilateral Tianshu,Shangjuxu,Jiexi,Diji,Zusanli,Zhongwan,and Daheng points of patients with critically ill gastrointestinal dysfunction can not shorten the target enteral nutrition compliance time,but can improve the gastrointestinal dysfunction score,AGI classification,TCM syndrome score,shorten the patient’s invasive mechanical ventilation time,improve the effect of nutritional support for critically ill patients,improve nutritional status,and promote disease recovery.There were no acupuncture-related adverse events in the two groups,indicating that the use of electroacupuncture to treat critically ill patients is safe.
