| Objective:The postoperative healing of anal fistula and postoperative healing rate were compared between the use of WuLinBai ointment and conventional dressing change to evaluate the effect of WuLinBai ointment on wound healing of various types of anal fistulas.Preliminarily establish a scientific evaluation method to make an objective and true evaluation of whether Wulinberry Ointment can promote wound repair after anal fistula operation.Analyze the influencing factors of postoperative recurrence of anal fistula,evaluate the recurrence of anal fistula in clinical work,and better guide clinical diagnosis and treatment.Methods:A single-center,retrospective cohort study was used to analyze the clinical data of 400 patients who underwent anal fistula surgery in the anorectal center of Nanjing Hospital of Chinese Medicine Affiliated to Nanjing University of Chinese Medicine from January 2019 to January 2020.According to the difference in the use of drugs for postoperative wounds,they were divided into the Cayratia japonica paste group(270 cases)and the conventional dressing change group(130 cases).Statistical analysis of the difference between the two groups of general information,hospital stay.In addition,the effects of gender,age,operation method,type of anal fistula,operation history of anal fistula and basic diseases(diabetes and hyperlipidemia)on the recurrence rate of postoperative anal fistula were analyzed in the two groups.Results:A total of 330 patients were followed up after exclusion of loss of follow-up due to various reasons.Among them,32 patients relapsed,with the recurrence rate of 9.7%.Among them,29 cases were in the Cayratia japonica ointment group and 3 cases were in the control group.According to the patient’s recovery after surgery after one-year follow-up,the recurrence rate of the patient in the Cayratia japonica ointment group was lower than that in the control group,and the difference was statistically significant.Analysis of influencing factors of postoperative recurrence of anal fistula showed that dressing change method(HR=0.229,95%Cl:0.108-0.484),Disease with infection(HR=2.958,95%Cl:1.051-8.323),Position of internal orifice(11-1PT HR=0.115,95%Cl:0.038-0.346;2-4PT HR=0.074,95%Cl:0.015-0.360;5-7PT HR=0.05,95%Cl:0.018-0.140;7-11PTHR=0.105,95%Cl:0.021-0.520;more points HR=0.376,95%Cl:0.097-1.450),Previous history of anal surgery(HR=2.257,95%Cl:1.182-4.709)were independent influencing factor of postoperative recurrence of anal fistula.Conclusion:1.The application of Cayratia japonica cream in postoperative management of anal fistula has the effect of reducing the postoperative recurrence rate,and is a good treatment choice for postoperative wound dressing of patients with anal fistula.2.Surgical method was the influencing factor of recurrence after anal fistula operation.Dressing changing method,BMI,whether accompanied by infection,position of internal orifice and previous anal operation history were the independent risk factors for recurrence of anal fistula. |
