| OBJECTIVE1.To construct a narrative nursing intervention program for Stroke-Related Stigma,so as to provide theoretical support for reducing stroke-related stigma.2.The narrative nursing intervention program will be applied to clinical practice,and its applicability and effectiveness will be discussed.METHODS1.Literature review:Searching the database worldwide and summarizing intervention strategies of stigma and narrative nursing,and to develop the study protocol.2.Delphi technique:Invite experts in related field to conduct two-rounds delphi inquiries.Determining the weights of each item and revising the intervention program according to the expert evaluations.3.Cross-sectional study:A convenience sampling method was used to select clinical nurse of one hospital in Guangzhou to conduct a questionnaire survey.The questionnaires include general information questionnaire、Narrative Competence Scale.Improving the content of the intervention staff and establishing the final narrative nursing intervention program.4.Quasi-experimental research:By adopting the method of quasi-experimental research.stroke patients are divided into two groups by non-contemporaneous control group method.The control group received routine care,while the intervention group carried out the intervention according to the narrative nursing intervention program constructed in this study.Stroke Stigma Scale and Self-Esteem Scale were used to evaluate the effect of intervention.5.Statistical methods:SPSS20.0 was used to analysis data.Response rate,Coefficient of authority,Kendall’s coefficient of concordance W were applied to examine the reliability of Delphi technique.In addition,mean and coefficient of variation were adopted to illustrate the evaluation of experts.Univariate and multivariate analyses were used to summarize the impact factors of narrative competence in clinical nursing.Two-sample t test was used to evaluate the effect of intervention programs.P<0.05 was considered as statistical significance.RESULTS1.Literature review:The stigma and narrative nursing intervention research includes 8 articles and 9 articles respectively.Summarizing 3 first-level items,14 second-level items and 57 third-level items according to these researches.2.Delphi technique:The recovery rates of the first and second rounds of effective expert consultation questionnaires are 100%and 93.8%,Coefficient of authority are 0.816,0.829,respectively.Kendall’s coefficient of concordance W of two rounds are statistical significance(P<0.05).3.Cross-sectional study:A total of 292 valid questionnaires were collected.According to the results of multi-factor analysis,two items were added and the narrative nursing intervention program was finalized.4.Quasi-experimental research:95 stroke patients were finally enrolled in the study.Before the intervention,there is no statistical significance in general information,stigma and self-esteem between the two groups(P>0.05).After the intervention,the stigma score of the intervention group is significantly lower than control group(P<0.05),and the self-esteem score of the intervention group is significantly higher than control group(P<0.05).CONCLUSION1.Based on literature review,delphi technique and cross-sectional study,this study constructs an intervention program including three dimensions of narrative nursing interventionists,narrative nursing intervention program,risk assessment,effect evaluation and quality control of narrative nursing.2.These results show that narrative nursing intervention can reduce stigma-related stigma and improve their self-esteem.clinical nursing can carry out narrative nursing intervention program to help stroke patients reduce their stigma. |
