Correlation Between Anxiety,Depression,Quality Of Life And Self-perceived Burden In Patients With Lumbar Disc Herniation

Abstract/Summary:
Objective : To understand the status quo of self-perceived burden,anxiety and depression,and quality of life of patients by investigating the self-perceived burden,anxiety,depression,and quality of life of patients with lumbar disc herniation.To study the relationship between self-perceived burden,anxiety,depression,and quality of life,as well as the influencing factors of self-perceived burden.Methods:In this study,300 patients with lumbar disc herniation who met the inclusion criteria from January 2020 to October 2020 in Ruikang Hospital Affiliated to Guangxi University of Chinese Medicine were selected by convenience sampling method as the investigation subjects.The research tools were general information questionnaire,self-perceived burden scale,hospital anxiety and depression scale,and the MOS 36-item short form health survey.The collected data was processed and analyzed by SPSS26.0,and count data was described by frequency,percentage,measurement data was described in((?)±S).After testing,the score of self-perceived burden obeys normal distribution,so T-test and analysis of variance were used to compare the difference of self-perceived burden between different groups.The relationship among anxiety and depression,quality of life and self-perceived burden was analyzed by Pearson correlation with bivariate following normal distribution.Taking the patient’s self-perceived burden as the dependent variable,and the variables with differences in univariate analysis as the independent variables,multiple linear regression analysis was performed.Results:(1)The effective respondents of this study were 283 cases,and the total score of self-perceived burden was(29.54 ± 6.75)points;among them,physical factors(14.76±2.99)points,emotional factors(11.88±3.82)points and economic factors(2.90±0.93)points.The level of self-perceived burden of patients was moderate,with no obvious self-perceived burden accounting for 7.0%,mild self-perceived burden accounting for 35.7%,moderate self-perceived burden accounting for 51.6%,and severe self-perceived burden accounting for 5.7%.(2)The average anxiety score of patients with lumbar disc herniation in the general hospital anxiety and depression scale was(9.19±2.35),indicating that anxiety was suspected.25.1% of the patients had no anxiety symptoms,40.3% of the patients were suspected to have anxiety symptoms,34.6% of the patients must have anxiety symptoms;The average score of depression was(8.79 ± 2.17),which indicated that depression was suspected.28.3% of the patients did not have depressive symptoms,42.0% of the patients were suspected to have depressive symptoms,and 29.7% of the patients definitely had depressive symptoms.(3)The scores of PF,RP,BP,GH,VT,SF,RE and MH in the SF-36 scale of patients were(36.02±14.20)points,(22.88±20.52)points,(31.77±12.93)points,(40.19±10.93)points,(37.42±9.33)points,(40.90±14.78)points,(40.13±28.20)points and(53.70±11.48)points,compared with the general population,are all worse in eight aspects(4)The single-factor variance results showed that the self-perceived burden of patients with lumbar disc herniation was statistically significant in age,per capita monthly income of the family,physical condition of the main caregivers,availability of medical insurance,duration of illness and pain degree(P<0.05).(5)The results of correlation analysis showed that self-perceived burden was positively correlated with anxiety(r=0.382,P<0.05),positively correlated with depression(r=0.320,P<0.05),and negatively correlated with PF、RP、BP、VT、SF、RE、MH of life quality(correlation coefficients were-0.154,-0.219,-0.255,-0.258,-0.361,-0.372,-0.502,P < 0.05).(6)The results of multiple linear regression analysis showed that patients’ age,family per capita income,the main caregivers,degree of pain,anxiety,physical condition and quality of life of the physiological function,social function,energy and emotional functions and mental health are the main factors of predicting self-perceived burden in patients with lumbar disc prolapse,which could explain 53.3% vzriztion of self-perceived burden.Conclusions :(1)Patients with lumbar disc herniation had moderate level of self-perceived burden,high incidence of anxiety and depression,and poor quality of life.Medical staff and their families need to provide comprehensive care and treatment,improve the quality patients’ life,eliminate the negative emotions,reduce the self-perceived burden.(2)The main influencing factors of the self-perceived burden of patients with lumbar disc herniation are age,per capita monthly income of the family,the physical condition of the main caregivers,availability of medical insurance,duration of illness and pain degree.Nursing patients should be given care according to the person.(3)The self-perceived burden of patients with lumbar disc herniation was positively correlated with anxiety and depression,and negatively correlated with quality of life.(4)Patients’ age,family per capita monthly income,physical condition of primary caregivers,pain degree,anxiety,physiological function,energy,social function,emotional function and mental health in quality of life are the main factors that predict the self-perceived burden of patients with lumbar disc herniation.
