Objective(s):To study the relationship between combined detection of plasma fibrinogen(FIB)level and serum albumin(Alb)level and overall survival(OS)of patients after gastric cancer surgery.Methods:The patients with gastric cancer(GC)admitted to the Second Clinical Hospital of Kunming Medical University from January 2014 to December 2 0 1 6,reviewed the relevant clinical medical records,including gender,age,tumor size,lymph node metastasis,TNM staging,albu min level,fibr in ogen level,tumor location and tumor cell differentiation.All patients were followed up by telephone and re-examination.The follow-up deadline was December 15,2020.According to the relevant inclusion and exclusion criteria,81 patients with gastric cancer were selected for D2.We drew the working characteristic curve(ROC curve)and calculated the optimal critical value of plasma albu min level to FAR)and albu min level in 8 1 patients,which were divided into high value group and low value groups,respectively.Through Alb and FIB level line FA score,high Alb low FIB score,high or low FIB score 1,low and high 2.By chi-square test,the relationship between albumin,FAR、FA score and fibrinogen and clinicopathological features was analyzed,univariate analysis Alb、FIB、relationship between clinicopathological features(including sex,age,tumor size,tumor cell differentiation,TNM stage,tumor location),FAR and FA scores and overall postoperative survival in gastric cancer patients,Drawing survival curves using kaplan-meier survival analysis,And use log-rank tests,And gradually into the COX proportion risk model to determine independent risk factors.Results:1.A total of 106 patients were followed up by telephone,and return to the hospital after surgery.According to the inclusion and exclusion criteria,a total of 25 patients were excluded.Among them,5 patients had both liver and kidney function abnormalities before surgery,and 6 patients TNM staging was stage Ⅳ without radical resection,5 patients received neoadjuvant radiotherapy and chemotherapy before operation,6 patients had missing data,and 3 patients were lost to follow-up after operation.Among 81 patients meeting the criteria,43 cases(53.09%)are alive and 38 cases(46.91%)are dead,with an average survival time of 40.78 months.2.The best cut-off values of albumin,fibrinogen and FAR are 35.25g/L,3.96g/L and 0.0974,respectively.3.The chi-square test showed that preoperative Alb level was correlated with age,tumor size,TNM stage of cancer,but not with sex,tumor location,lymph node metastasis and tumor cell differentiation;preoperative FIB level was related to lymph node metastasis,sex and tumor location,but not with tumor cell differentiation,age group,cancer TNM stage and tumor size;The size of the FAR was related to sex and lymph node metastasis,not to tumor size,tumor location,different age groups,tumor cell differentiation degree and TNM stage.The FA score was not related to TNM stage,tumor size,sex,age group,tumor location,lymph node metastasis and tumor cell differentiation.4.Use Kaplan-Meier to calculate survival curve,and use log-rank test for univariate analysis to show that tumor size,lymph node metastasis,tumor cell differentiation,TNM stage,albumin level,fibrinogen level,FAR and FA scores are all related to gastric cancer Postoperative overall survival is significantly related;COX univariate analysis and progressive multivariate COX regression show that the degree of tumor cell differentiation,albumin level,fibrinogen level,FA score and FAR size can all become the overall survival of gastric cancer patients after surgery Independent risk factors for the period.Conclusion(s):1.Albu min level grouping,fibr in ogen level grouping,and FAR、FA scores were all significantly associated with overall survival after gastric cancer.2.The FA score is an independent risk factor affecting overall postoperative survival in patients with gastric cancer.
