Abstract/Summary:
Objective: This study used three-dimensional power Doppler ultrasound to retrospectively study the placental blood flow parameters(VI,FI,VFI)of the same pregnant woman in the second and third trimesters,and explore the changes of placental blood flow parameters(VI,FI,VFI).Changes in gestational age,measurement of placental VI,FI,VFI in different gestational weeks of implantation group and non-implantation group,provide reliable basis for the diagnosis of placental implantation,normal and abnormal placental function.Methods: Randomly collected pregnant women who were filed in our hospital’s outpatient clinic for prenatal examination from October 1,2019 to December 31,2020.All pregnant women underwent three-dimensional power Doppler ultrasound examinations during the second and third trimesters,and collected three-dimensional volume data of areas found in grayscale and color patterns or suspected placenta accretion.After collection,the data is stored in the instrument workstation and the VOCAL software is used Analyze the image.Use the three-dimensional energy histogram to automatically calculate the VI,FI and VFI of the placenta.According to the surgical and(or)pathological diagnosis of placenta accreta and the size of the gestational age at the time of examination,they are divided into: 1)second trimester placenta accreta group and second trimester no placenta accreta group;2)third trimester placenta accreta group And third trimester without placenta accreta group;3)second trimester placenta accreta group and third trimester placenta accreta group;4)second trimester placenta accreta group and third trimester without placenta accreta group.SPSS25.0 software was used for data analysis.The measurement data is expressed by((?)±s),and the independent sample t test is used for comparison between groups;the paired sample t test is used for the comparison of different gestational weeks;the correlation between the two samples is analyzed by Pearson correlation;the count data is expressed by n(%),The χ2 test was used for comparison between groups;the ROC curve was used to evaluate the diagnostic efficacy of placental implantation and nonimplantation in different gestational age groups,and the AUC was calculated.The diagnostic threshold was determined according to the highest critical point of the Youden index combined with the actual clinical application,P<0.05 indicated There is statistical significance.Results:1.The VI,FI,VFI of normal placenta were positively correlated with gestational age,and the value increased with the increase of gestational age,and the correlation between VI and VFI and gestational age was better than FI;2.The VI,VFI and FI of the placenta accreta in the second and third trimester groups were higher than those in the placenta accreta group in the second and third trimesters,and all parameters were statistically significant(P<0.01),VI,VFI,and FI were three Both can be applied to the diagnosis of placenta accreta;3.The VI and VFI of the placenta implantation group in the third trimester were higher than those in the second trimester implantation group,but the difference in FI was not significant(P>0.05),indicating that VI and VFI have more diagnostic value than FI during the placenta implantation in the third trimester.4.The VI,FI and VFI of the third trimester without placenta accreta group were higher than those of the second trimester without placenta accreta group,with statistical significance(P<0.05).Conclusion: 1.Three-dimensional power Doppler provides a new and effective method for quantitative observation of placental blood perfusion,which can evaluate placental blood perfusion and provide a basis for clinical evaluation of placental implantation.2.Three-dimensional power Doppler blood flow parameters VI,FI,and VFI are increased and increased in the implantation group during the second trimester,providing a reliable objective basis for early detection of placental implantation,and it also has a certain predictive value for implantation grade;3.Three-dimensional power Doppler can improve the level of prenatal diagnosis of placenta accreta.
