Objective:(1)To understand the status of anxiety and depression in elderly inpatients;(2)To analyze the characteristics of heart rate variability in elderly patients with negative emotions and explore its significance.Methods:(1)A total of 287 elderly inpatients and 60 elderly healthy subjects who were hospitalized in the Department of Geriatrics of North Sichuan Medical College from October 1,2019 to November 31,2020 were selected as the subjects;(2)Record the general information of each object and improve the scale evaluation,Classification into anxiety group(group A,n=80),the depression group(group B,95 cases),comorbid group(group C,112 cases)and control group(group D,60 cases),After fasting,5ml of venous blood was extracted from all subjects on an empty stomach for the detection of relevant laboratory indicators;.(3)All the subjects completed Holter,HR,SDNN,RMSSD,SDANN,HF,LF and LF/HF were recorded for total time,daytime(08:00-22:00)and night(22:00-08:00),respectively,and SPSS 22.0 was used for analysis.Results:(1)About 42.0%of elderly hospitalized patients were associated with mood disorders,with a mere anxiety of 11.0%,with a total of 12.3%depression and 18.7%anxiety and depression;(2)General data and routine test results showed that the incidence of sleep disorders in group A,B and C was significantly higher than that in group D(P>0.05),and there was no difference in other items between groups(P>0.05).(3)There were significant differences in HRV indexes among all groups except SDNN(P<0.05).The average HR、LF/HF(24h)of group D was significantly lower than that of the other groups(P<0.05);SDANN(24h)of group B was significantly lower than that of group D(P<0.05);In contrast to LF(24h),the level of RMSSD(24h)in group D was significantly higher than others(P<0.05 or P<0.01).The HF(24h)of group C was significantly lower than others(P<0.05 or P<0.01).(4)The mean HR of females in group C was significantly higher than that of males(P<0.01);In contrast to SDANN,RMSSD and HF indexes of female group A,B and C were significantly higher than those of male group(P<0.01).The SDNN of male in group A and D was significantly higher than female(P<0.01 or P<0.05).LF level of male was significantly higher than female in all groups(P<0.01 or P<0.05),LF/HF of male in group A was significantly higher than female(P<0.01).(5)In contrast to average HR and LF/HF,RMSSD and HF were increased at night(P<0.01 or P<0.05).Except group B,the level of SDANN in other groups decreased at night,but the level of LF increased(P<0.05),SDANN in group A was significantly higher than that in group D during day and night(P<0.01).,and the RMSSD level in group D was significantly higher than others day and night(P<0.01).The LF level in group D was lower than that in group A in day and night(P<0.01 or P<0.05).Contrary to HF,LF/HF in group C was significantly higher than that in groups B and D(P<0.05 or P<0.01).Conclusion:(1)The incidence of mood disorders in elderly inpatients in our hospital may be higher than before;(2)Sympathetic tension increased and parasympathetic level decreased in elderly patients with mood disorders;Anxiety and depression may have a superposition effect on the influence of HRV,and depression has a stronger inhibitory effect on parasympathy;(3)Women may be more resilient to mood disorders in old age than men;(4)HRV has a circadian rhythm,and parasympathetic tension increases while sympathetic tension weakens at night,and negative emotions may be one of the reasons for HRV rhythm disorder.
