Application Of Clinical Nursing Path Under The ERAS Concept

Abstract/Summary:
Objective:To explore the impact of clinical nursing pathways under the concept of accelerated recovery after surgery(ERAS)of lumbar spinal stenosis patients undergoing lumbar intervertebral fusion and internal fixation.Our research studied lumbar spine function,postoperative pain,the postoperative complications,the length of hospital stays,hospitalization costs and patient satisfaction with nursing care,which provides theoretical basis for clinical nursing work.Methods:A total of 100 patients were randomly selected from Januaryl,2018 to July31,2018 and Januaryl,2019 to July 31,2019 in a second-level hospital for lumbar spinal stenosis due to lumbar spinal stenosis,including control group and intervention group.The control group performed perioperative nursing according to the routine nursing path,the intervention group performed perioperative nursing according to the clinical nursing path under the ERAS concept.We observed and recorded lumbar spine function recovery,postoperative pain score,the postoperative complications,perioperative hospital stay,hospitalization cost and patient satisfaction with nursing work.We compared the differences between the two groups of patients during the perioperative period.Results:1.The implementation of the clinical nursing pathway under the ERAS concept has improved the lumbar spine function recovery in patients with lumbar spinal stenosis undergoing lumbar interbody fusion surgery.On the day of admission,there was no statistical difference in JOA scores between the two groups of patients P>0.05.The postoperative JOA scores of patients in the intervention group were significantly higher than those in the control group who implemented the routine nursing route.There was a significant statistical difference between the two groups,P<0.05.2.The implementation of clinical nursing pathway under the ERAS concept has a more obvious effect on reducing postoperative pain of patients.On the day of admission,there was no statistical difference in VAS scores between the two groups of patients P>0.05.Compared with the control group on the 1st,2nd,and 3rd days after the operation,the implementation of the clinical nursing pathway under the ERAS concept had a more significant effect on reducing postoperative pain.The VAS scores of the two groups were statistically different,P<0.05.3.Compared with the control group,the incidence of urinary retention and urinary tract infections,abdominal distension and lung infections in the intervention group was significantly lower than that in the control group.The difference was statistically significant,P<0.05.However,the incidence of cerebrospinal fluid leakage,deep vein thrombosis of lower limbs,and wound infections were not statistically different,P>0.05.4.The implementation of clinical nursing pathway under the ERAS concept has statistical significance in shortening the perioperative hospitalization time and reducing the hospitalization cost of patients,P<0.05.5.The application of the clinical nursing pathway under the ERAS concept in the perioperative period of lumbar intervertebral fusion and internal fixation surgery for patients with lumbar spinal stenosis has improved patient satisfaction,the difference is statistically significant,P<0.05.Conclusions:The standardized application of the clinical nursing path under the ERAS concept in the perioperative period of lumbar intervertebral fusion internal fixation surgery for patients with lumbar spinal stenosis can promote the recovery of lumbar spine function,relieve the pain after surgery.It can reduce the incidence of postoperative complications,shorten the number of patients during the perioperative period,reduce the cost of hospitalization,and improve the patient’s satisfaction with nursing work.It is worthy of clinical research.
