Objective With aging process continuously accelerating,the prevention and control of chronic diseases in the elderly has brought great challenges to the development of the society.This study analyses the relationship between thyroid hormone and benign prostatic hyperplasia(BPH).Methods Based on the data of population over 60 years old and 65 years old from National Bureau of Statistics,this study analyses the growth trend over past 20 years.80 elderly patients suffering from BPH were selected from the health care department of General Hospital of Tianjin Medical University from January 2018 to December 2019.The results were compared with 80 age-matched controls.Multiple factors logistic regression was used to analyze the connection between FT3,FT4,TSH and BPH.The data of prostate volume(PV)were used as the index of clinical progression of BPH.The BPH patients with PV over 30 ml were named high progressive group,and logistic regression was used to explore the relationship between FT3,FT4,TSH and clinical progression of BPH.Results 1.The population over 60 years old in China has increased from 130 million in 2000 to 254 million in 2019.The proportion of the population over 60 years old has increased from 10.32% to 18.10%.The average annual growth rate of the elderly population in the past 20 years is 3.59%,much higher than the average(2.7%)over the world.At the same time,the population over the age of 65 years old also increased from 90 million in 2000 to 176 million in 2019,and the proportion also increased from 6.96% to 12.60%.2.Single factor Logistic regression analysis has found that TSH is a protective factor of BPH [OR = 0.776,(95% CI 0.609-0.989)];FT3 is a protective factor of BPH [OR = 0.546,(95% CI 0.356-0.836)];FT4 is not correlated with BPH [OR = 1.137,(95% CI 0.952-1.357)];LDL is a protective factor for the onset of BPH [OR = 0.692,(95% CI 0.485-0.987)];PSA is a risk factor for onset of BPH [OR = 1.455,(95% CI 1.074-1.972)].After adjusting for confounding factors such as age,PV,PSA,and LDL,it was found that FT3 is an independent related factor for the onset of BPH [OR = 0.588,(95% CI 0.369-0.937)] and there is still a significant correlation between TSH and the incidence of BPH [OR = 0.762,(95% CI 0.587-0.988)].3.Taking the risk of clinical progression of BPH patients as the dependent variable,the relationship between serum TSH,FT3,FT4 levels and clinical progress of BPH and related risk factors of clinical progress of BPH were analyzed by Logistic regression model.There is no significant relationship between TSH,FT3,FT4 and clinical progression of BPH(P > 0.05);DBP [OR = 1.080,(95% CI 1.023-1.140)],TC [OR = 1.631,(95% CI 1.010-2.634)],LDL [OR = 1.980,(95% CI 1.092-3.588)],and PSA [OR = 1.980,(95% CI 1.092-3.588)] are risk factors of the clinical progression of BPH.After adjusting for confounding factors such as age,T,TC,LDL,multivariate logistic regression analysis has shown that PSA is an independent risk factor for the clinical progress of BPH [OR = 2.039,(95% CI 1.135-3.666)].4.In elderly men,taking PV as the dependent variable,linear correlation analysis has shown that PV is positively correlated with DBP(r = 0.191),TC(r = 0.179),HDL(r = 0.178),LDL(r = 0.156)(P <0.05),and PV is significantly positively correlated with PSA level(r = 0.456)(P <0.01);PV is not significantly correlated with TSH,FT3,and FT4(P > 0.05);moreover,step-wise linear regression has found that PSA is an independent positive factor for PV(standardized regression coefficient is 0.456,P = 0.000).When the PSA level increases by 1.07 ng / m L,PV will increase by 6.3ml.5.In elderly BPH patients,taking PV as the dependent variable,linear correlation analysis has shown that PV is significantly positively correlated with DBP(r = 0.347)and PSA(r = 0.385)(P <0.01);and TSH,FT3,and FT4 are not correlated with PV(P> 0.05);furthermore,PSA has been found to be positively correlated with PV by stepwise linear regression analysis(standardized regression coefficient was 0.493,P = 0.000).When the PSA level increases by 1.17 ng / m L,PV will increase by 7.24 ml.Conclusion In China,the average annual growth rate of the elderly population in the past 20 years was much higher than the average over the world.TSH and FT3 of elderly men are closely related to the incidence of BPH;FT4 is not correlated with BPH;With the increase of TSH and FT3 levels,the incidence of BPH went down.There is no relationship between TSH,FT3,FT4 and clinical progression of BPH.The increase of TSH and FT3 levels in the physiological range may help to prevent BPH.
