| Objective:1.According to the diagnostic criteria of postpartum stress urinary incontinence, selecte the subjects, explore the rehabilitation methods and influencing factors of postpartum stress urinary incontinence, and provide the new ideas and options for women’s health care to prevent the occurrence of postpartum stress urinary incontinence.2.To compare the clinical efficacy of breathing training with sling exercise therapy on postpartum stress urinary incontinence,so as to provide a safe and efficient treatment method for the conservative treatment of postpartum stress urinary incontinence.Method:Taking primiparas diagnosed with stress urinary incontinence(SUI)after 6weeks of postpartum reexamination at the outpatient department of obstetrics and gynecology and postpartum rehabilitation center of Jiangdu People’s Hospital in Yangzhou city from January 2018 to April 2019 as the research objects, the rehabilitation methods and influencing factors of postpartum stress urinary incontinence were explored. Then, after signing the informed consent,198 patients were selected and numbered, divided into routine group, sling group and the breathing group according to age, body mass index, body mass of newborn and so on.The routine group used pelvic floor muscle exercise(PFMT),including simple Kegel pelvic floor muscle training method and intermittent urination training method. The sling group was supplemented with sling exercise therapy on the basis of the routine group, and the breathing group was supplemented with breathing training on the basis of the routine group.Three groups of patients were evaluated before and 12 weeks after treatment.Pelvic floor electromyography, urinary incontinence frequency,1-hour urine pad test and average thickness index of transverse abdominal muscle were used for evaluation. Results:In the routine group, sling group and breathing group,5,6 and 6 patients respectively fell off due to incomplete training plan requirements,so 181 patients were finally treated,61,60 and 60 patients respectively.Compared with previous treatment, pelvic floor electromyography, urinary incontinence frequency and 1-hour urine pad test index are better in the routine group, sling group and breathing group at12 weeks after treatment, there was significant difference before and after the treatment(P<0.05),compared with the routine group, the sling group and the breathing group were superior in terms of electromyography value of pelvic floor muscle, frequency of urinary incontinence and 1-hour pad test index, with significant difference(P<0.05),while the sling group and the breathing group showed no significant difference(P>0.05);Compared with previous treatment, the sling and breathing group at 12 weeks after treatment, the transverse abdominal muscle is obviously better than the average thickness before treatment, significant differences(P<0.05),the routine group at 12 weeks after treatment, the average transverse abdominal muscle thickness no difference compared with previous treatment, there was no significant difference(P>0.05),however,sling and breathing group in the treatment of 12 weeks after treatment, the average transverse abdominal muscle thickness has significant difference(P<0.05),breathing group is better than the sling group(P<0.05).Conclusion:1.The effect of routine training combined with sling exercise or breathing training on postpartum stress urinary incontinence is better than the routine training alone.2.Core exercise therapy(sling exercise and breathing training)has the advantages of easy operation, low cost and strong acceptability, which can be applied to the routine rehabilitation treatment of postpartum stress urinary incontinence. |
