Correlation Between Bone Turnover And Glucose And Lipid Metabolism In Patients With Phlegm Dampness Syndrome Of Type 2 Diabetes Mellitus With Different Bone Mass

Abstract/Summary:
Objective: To observe the fasting blood glucose(FBG),fasting C peptide(FCP),glycosylated hemoglobin(HbA1c),total cholesterol(TC),triglyceride(TG),low density lipoprotein(LDL),high density lipoprotein(HDL),alkaline phosphatase(ALP)and bone turnover index(BALP),osteocalcin(OC)in patients with phlegm dampness syndrome of type2 diabetes mellitus(T2DM)with different bone mass)To analyze the correlation between glucose and lipid metabolism,bone metabolism and bone mineral density(BMD)in patients with type 2 diabetes mellitus(T2DM)of phlegm dampness syndrome.Methods:110 cases of T2 DM patients with phlegm dampness syndrome were collected.The bone mineral density(BMD)was measured by dual energy X-ray absorptiometry(DEXA).According to the results of T value,the patients with t ≥-1.0sd were included in the normal bone mass group(n = 36),the patients with t value between-1.0sd and-2.5SD were included in the osteopenia group(n = 37),and the patients with t ≤-2.5SD were included in the osteoporosis group(n = 37).The general information(gender,age,course of diabetes and complications)of the three groups were collected,and the indexes of glucose metabolism(FPG,FCP,HbA1c),lipid metabolism(TC,TG,LDL,HDL),ALP and bone turnover(BALP,OC,PINP)were measured.The differences of the above indexes among different bone mass T2 DM patients with phlegm dampness syndrome and the correlation between glucose and lipid metabolism indexes,bone metabolism indexes and bone mineral density in T2 DM patients with phlegm dampness syndrome were analyzed.Results: 1.A total of 110 cases of T2 DM patients with phlegm dampness syndrome were collected in this study,including 36 cases of normal bone mass group,37 cases of osteopenia group and 37 cases of osteoporosis group.There was no significant difference in gender,age,course of disease and diabetic complications among the three groups.FPG,FCP and Hb A1 c of three groups were compared.Hb A1 c of osteoporosis group was higher than that of normal bone mass group and osteopenia group.There was no significant difference between normal bone mass group and osteopenia group.There was no significant difference in FBG and FCP among the three groups.TC,TG,LDL and HDL of three groups were compared.Tg of osteoporosis group was significantly higher than that of normal bone mass group,Tg of osteopenia group was higher than that of normal bone mass group,Tg of osteoporosis group was higher than that of osteopenia group,but there was no statistical difference between the two groups.There was no significant difference in TC,LDL and HDL among the three groups.There was no significant difference in ALP among the three groups.Lumbar l1-l4 bmd and femoral neck BMD were compared among the three groups.Osteoporosis group < osteopenia group < normal bone mass group.The OC among the three groups was osteoporosis group >osteopenia group > normal bone mass group.The PINP of osteoporosis group was higher than that of osteopenia group and normal group.2.Correlation analysis: L1-L4 BMD,femoral neck BMD and FCP were positively correlated.L1-L4 BMD and femoral neck BMD were negatively correlated with Hb A1 c.There was no correlation between L1-L4 BMD,femoral neck BMD and FBG.L1-L4 BMD and femoral neck BMD were negatively correlated with TG.There was no correlation between L1-L4 BMD,femoral neck BMD and TC,LDL,HDL.There was no significant correlation between L1-L4 BMD,femoral neck BMD and ALP.L1-L4 BMD and femoral neck BMD were negatively correlated with OC and PINP.Conclusion: 1.There is correlation between BMD and FCP,HbA1cTG,OC,PINP in T2 DM patients with phlegm dampness syndrome.2.The bone loss of T2 DM patients with lower FCP and higher Hb A1 c and TG is more serious.3.In T2 DM patients with phlegm dampness syndrome,OC and PINP will increase when osteoporosis occurs.
