Analysis Of Therapeutic Effect And Endotoxin Of "Tongfu Xiedu Decoction" On Acute Pancreatitis With Intra-Abdominal Hypertension (Yang Ming Fu Shi Syndrome)

Objective:To analyze the curative effect and endotoxin effect of Tongfu Xiedu decoction on acute pancreatitis with abdominal hypertension in order to provide experimental basis for its clinical application and popularization.Methods:78 patients with acute pancreatitis with abdominal hypertension were randomly divided into experimental group(n=70)and control group(n=35).On the basis of comprehensive treatment of western medicine,the experimental group was treated with Tongfu Xiedu decoction enema,while the control group was treated with rhubarb powder enema,the course of treatment is 7 days.The changes of abdominal pressure,endotoxin,the integral of TCM syndrome,the change rate of intra-abdominal pressure and the curative effect of TCM syndrome before and after treatment of these two groups were statistically analyzed.Results:(1)A total of 78 cases were included in this study,4 cases were lost in the experimental group and 4 cases in the control group.A total of 70 people completed the observation of this study,the drop rate was 10.3%.(2)Comparable analyzed all the research objectives,there was no significant difference in sex distribution,age distribution and etiology distribution between the two groups(P>0.05).(3)After treatment,there was significant difference in the curative effect of TCM symptoms between the two groups(P<0.05).The total score of TCM symptoms in the two groups was lower than that in this group before treatment,and the difference was statistically significant(P<0.001).There were significant differences in the scores of"abdominal pain","abdominal distension","constipation"and"hot flashes"between the two groups before and after treatment(all P<0.05).There was no significant difference in the scores of"vomiting","epigastric obstruction"and"short red urine"in the control group before and after treatment(P>0.05).There were significant differences in the scores of"abdominal pain","vomiting"and"epigastric obstruction"between the two groups before and after treatment(P<0.05).(4)After treatment,the abdominal pressure in both groups was lower than that in this group before treatment,and the difference was statistically significant(P<0.001).There were also significant differences in abdominal pressure difference and intra-abdominal pressure change rate between the two groups before and after treatment(P<0.05).(5)After treatment,the level of endotoxin in the two groups was lower than that before treatment,and the difference was statistically significant(P<0.001).There was also significant difference in the level of endotoxin between the two groups before and after treatment(P<0.05).(6)Before treatment,there was a positive correlation between intra-abdominal pressure and endotoxin in treatment group(r_s=0.904,P<0.001),and between intra-abdominal pressure and endotoxin in control group(r_s=0.887,P<0.001).(7)During the treatment,there were no serious adverse reactions in both groups.Conclusion:Retention enema of Tongfu Xiedu decoction can reduce abdominal pressure and endotoxin level in patients with acute pancreatitis with abdominal hypertension,improves TCM symptoms and has no side effects.
