Objective:By observing the difference of therapeutic effect of acupuncture Zhongfu(LU1),Yunmen(LU2)and external diaphragm pacemakers(EDP)on patients with respiratory dysfunction after intracerebral hemorrhage,and to provides a new idea and method for the treatment of respiratory dysfunction after intracerebral hemorrhage by acupuncture.And explore the possible mechanism of acupuncture at zhongfu and Yunmen for improving respiratory function of patients.Methods:74 patients with respiratory dysfunction after cerebral hemorrhage who met the inclusion criteria were divided into control group and observation group,37 patients in each group,all of them were given rehabilitation training of pulmonary function and basic treatment.The observation group was treated with zhongfu and Yunmen acupuncture from Monday to Saturday,30 min/time for 2 weeks.The control group was treated with external diaphragm pacemaker from Monday to Saturday,30 min/time for 2 weeks.The changes of diaphragm thickness(DT),diaphragm thickening fraction(DTF),diaphragmatic excursion(DE),PaCO2,oxygenation index(OI)and tracheostomy cough reflex grading score(TCRGS)were ob-served before and 2 weeks after treatment,and evaluate the therapeutic effect and weaning success rate of the two groups.The data were statistically analyzed by SPSS 22.0.Result:A total of 74 cases were included in the trial,71 cases completed the clinical trial observation,3 cases were not completed,1 case was excluded from the observation group,and2 cases fell off from the control group.1.There was no significant difference in the basic data between the two groups(P>0.05).2.Objective indicators:(1)Diaphragm function:After the end of treatment,the DT,DTF and DE improved in both groups compared with those before treatment(P<0.01),and the difference between the observation group and the control group was not statistically significant(P>0.05).(2)PaCO2:After treatment,PACO2was improved in both groups(P<0.01),and there was significant difference between observation group and control group(P<0.05).(3)Oxygenation index:At the end of treatment,the OI improved in both groups compared with that before treatment(P<0.01),and the difference between the observation group and the control group was not statistically significant(P>0.05).(4)Tracheostomy cough reflex grading score:At the end of treatment,the TCRGS improved in both groups compared with that before treatment(P<0.01),and the difference between the observation group and the control group was statistically significant(P<0.05).(5)Serum prealbumin(PA):After the end of treatment,the PA improved in both groups compared with that before treatment(P<0.01),and the difference between the observation group and the control group was statistically significant(P<0.05).3.The successful rates of weaning from mechanical ventilation:After 2 weeks of treatment,the success rates of the observation group were 69.4%,and the control group were 45.7%,the difference was statistically significant(P<0.05).4.Therapeutic effect:After 2 weeks of treatment,the total effective rate of the observation group and the control group was 94.4%and 88.6%respectively,the difference was statistically significant(P<0.05).Conclusion:(1)Acupuncture of Zhongfu,Yunmen and EDP therapies both improved respiratory function in patients with respiratory dysfunction after cerebral hemorrhage.There was no difference between the two groups in terms of improving DT,DTF,DE and OI.(2)And acupuncture of Zhongfu and Yunmen was more effective in terms of improving PaCO2,TCRGS,PA,successful rates of weaning from mechanical ventilation and therapeutic effect.(3)Acupuncture Zhongfu and Yunmen could effectively improve the mortality rate and overall efficacy of patients with respiratory dysfunction after cerebral hemorrhage,which is better than EDP therapies.
