| Objective:By retrospectively analyzing the clinical characteristics and laboratory characteristics of patients with polycythemia vera(PV),the cognition of clinicians on PV was improved.Methods:The clinical and laboratory data of 56 patients with definite diagnosis of PV in Department of Hematology,People’s Hospital of Ningxia Hui Autonomous Region from January2008 to December 2020 were collected.The general data include age and gender.Clinical data included dizziness,headache,numbness of limbs,pruritus,thrombus history,history of hypertension,hepatosplenomegaly,etc.The laboratory data include white blood cell(WBC),hemoglobin(Hb),platelet(PLT),coagulation function,serum erythropoietic hormone(EPO)level,JAK2 exon12-15 mutation,vitamin B12,folic acid,etc,and their characteristics were summarized and analyzed.56 patients was divided into thrombotic group and non-thrombotic group according to thrombotic events,and risk factors related to thrombotic events were analyzed statistically.Results:(1)Clinical features:In this study,the median age of PV patients was 65.5 years old,and the ratio of male to female patients was nearly 2:1.Thrombosis events occurred in 18 patients(32.1%).There were 37 cases of splenomegaly(66.1%)and 5 cases of hepatomegaly(8.9%).35patients(62.5%)had hypertensive disease.Acute myeloid leukemia(AML)occurred in 4 cases(7.1%).(2)Laboratory results:the median white blood cell count was 12.44(1.97-42.34)×10~9/L,the median hemoglobin concentration was 191.0(23.3-238.0)g/L,and the median platelet count was 339.5(4.0-889.0)×10~9/L at the time of consultation.Among the 56 PV patients,54 cases had JAK2V617F mutation,1 case had JAK2 exon12 mutation,and 1 case had JAK2 exon13mutation.(3)Patients with thrombosis and non-thrombus group PV in gender,age,spleen,liver,hyperlipidemia,high uric acid hematic disease,white blood cells(WBC),hemoglobin(Hb)and platelets(PLT)and erythropoietin(EPO),prothrombin time ratio(PTR),activated partial prothrombin time(APTT),thrombin time(TT),D-dimer(D-D)aspects of no statistical significance(P>0.05).Logistic regression analysis of risk factors for PV thrombotic events excluding the influence of other factors showed that hypertension and TT were independent risk factors for thrombotic events(P<0.05),and hypertension and TT were classified according to the OR value.Conclusion:(1)Hypertension and TT were risk factors for PV thrombotic events.(2)PV diagnosis only detects JAK2V617F mutation,which may miss PV patients with JAK2 exon12and JAK2 exon13 mutation. |
