Meta-analysis Of The Predictive Value Of Systemic Immune Inflammation Index In The Prognosis Of Breast Cancer Patients

Abstract/Summary:
Objective Breast cancer is a heterogeneous tumor with different proliferation rate,invasion ability,metastasis potential and therapeutic effect.Although there are a variety of treatment methods at present,a large proportion of breast cancer patients will eventually die of recurrence and metastasis of breast cancer.Therefore,sensitive prognostic markers are particularly important for breast cancer patients.The purpose of this meta-analysis was to investigate the predictive value of systemic immune-inflammation(SII)index in the prognosis of breast cancer patients.Methods: All the studies on the correlation between SII index and prognosis of breast cancer patients from January 2015 to July 2020 were collected by searching Web of Science,Pub Med,Embase,Cochrane Library,CNKI and Wan FANG databases,in which SII index was defined as platele × neutrophil/ lymphocyte counts(P×N/L).The two researchers independently screened the literature according to the inclusion and exclusion criteria,evaluated the quality of the final included literatures and extracted relevant data.The extracted data were combined and statistically analyzed using Stata12.0 and Review Manager 5.3 software.The odds ratio(OR)and 95% confidence interval(CI)were used to assess the relationship between SII index and pathological parameters(tumor size and lymph node status)of breast cancer patients.The harzard ratio(HR)and 95% CI were used to evaluate the association between SII index and overall survival(OS),disease-free survival(DFS)in breast cancer patients.Results: A total of 8 studies were included in this study,including 2642 patients with breast cancer.The primary outcome measures were tumor size,lymph node status,OS,and DFS.Meta analysis results showed that breast cancer patients with high SII index had a higher incidence of tumor diameter > 2cm than patients with low SII index(combined OR=1.49,95%CI: 1.19-1.85,P = 0.0004,fixed effect model);Breast cancer patients with high SII index had a higher incidence of lymph node metastasis than patients with low SII index(combined OR=1.38,95%CI: 1.12-1.69,P=0.002,fixed effect model);The OS(combined HR=2.06,95%CI: 1.55-2.74,P < 0.00001,random effect model)and DFS(combined HR=1.93,95%CI: 1.63-2.30,P < 0.00001,fixed effect model)of patients with high SII index were shorter than those with low SII index.Conclusions: SII index can be used as a reliable indicator to predict the prognosis of breast cancer patients.
