| Objective: Evaluation of the clinical efficacy and safety of Chinese herbal thumb jars in ankylosing spondylitis(kidney deficiency and cold syndrome).Methods:174 patients admitted to the rheumatology and orthopaedic disease centre of a tertiary hospital in Gansu Province from December 2019 to October 2020 with kidney deficiency and cold syndrome AS were included,Randomly divided into test group,control group 1 and control group 2,58 cases in each group.In the test group,basic treatment +Chinese herbal thumb jar therapy was used: the formula used for boiling the jar was based on the "Formula for the Treatment of Bai Shui Hou of the East China Sea" with an addition and subtraction,including 10 g of Ox Knee,10 g of wine cornelian,10 g of Phellodendron,10 g of Phellodendron,10 g of Cistanches,10 g of Radix Platycodon,10 g of Cinnamomum,5g of Hei Shun Pian,15 g of Wei Ling Xian,10 g of Radix Rehmanniae,10 g of Radix Fructus,10 g of Radix Fructus,10 g of Gingerwood,15 g of Dou Wu,10 g of Branched Yam.The cupping points were the back acupoints of the Directing Vessel and the Bladder meridian,and the cupping was left for 10 min.in the control group,the cupping points were the indicators(thoracic mobility,occipital wall distance,Schober test),laboratory indicators(CRP,ESR)and the rate of achieving ASAS and the total efficacy rate of TCM symptoms before and after 3 courses of treatment in the three groups,and recording the patients’ adverse reactions,the treatment of Chinese medicine thumb jar was Evaluation of the efficacy of ankylosing spondylitis.Results:1.Research objects: 174 cases were included in the trial,4 cases were dislodged and 2cases were excluded during the treatment,and 168 valid statistical cases were obtained,Shedding rate less than 20%.2.Comparison of general information: There was no significant difference in age,gender and duration of illness between the three groups at the time of enrolment(p>0.05),which was comparable.3.Clinical efficacy(1)BASDAI: After three courses of treatment,the BASDAI scores of all three groups were lower than before treatment,and the difference was statistically significant(P < 0.05).The comparison between the three groups showed that the test group was better than the control group 1 and the control group 2(P < 0.05),indicating that the patients in the test group had better improvement in BASDAI scores than the control group 1 and the control group 2.(2)BASFI: After three courses of treatment,the BASFI scores of all three groups were lower than those before treatment,and the difference was statistically significant(P < 0.05).When comparing the three groups,the test group was better than the control group 1 and the control group 2(P < 0.05),and the control group 1 was better than the control group 2(P <0.05),indicating that the patients in the test group had better improvement in BASFI scores than the control group 1 and the control group 2,and the control group 1 was better than the control group 2.(3)Chinese medicine evidence points: After three courses of treatment,the Chinese medicine evidence points of all three groups decreased compared with those before treatment,and the difference was statistically significant(P<0.05).(4)Occipital wall distance and thoracic mobility: After three courses of treatment,the thoracic mobility and occipital wall distance in all three groups improved compared with those before treatment,and the difference was statistically significant(P < 0.05),indicating that all three treatments could improve the patients’ thoracic mobility and occipital wall distance.(5)Schober test: After three courses of treatment,the Schober test value in the test group only improved compared with that before treatment,and the difference was statistically significant(P < 0.05),indicating that the treatment method in the test group only could improve the Schober test value of patients.(6)CRP and ESR: The CRP and ESR in all three groups decreased after three courses of treatment compared with those before treatment,and the difference was statistically significant(P < 0.05),indicating that all three treatments could reduce the CRP and ESR values of patients.(7)ASAS attainment rates: the ASAS20 attainment rates of the three groups were90.91%,63.16% and 48.12% respectively;the ASAS50 attainment rates of the three groups were 63.64%,31.58% and 17.86% respectively;the ASAS70 attainment rates of the three groups were 18.18%,3.51% and 0.00% respectively,indicating that the patients in the experimental group had better ASAS The achievement rate was better than that of the control group 1 and the control group 2.(8)Efficacy of Chinese medicine symptoms: the total effective rate of Chinese medicine symptoms in the test group,the control group 1 and the control group 2 were 92.73%(51/55),82.45%(47/57)and 71.43%(40/56)respectively,indicating that the total effective rate of Chinese medicine symptoms in the test group was better than that in the control group 1 and the control group 2.4.Adverse reactions: There were 1,4 and 2 cases of small blisters in the test group,the control group 1,and the control group 2 respectively during the treatment.The blisters were absorbed by themselves the next day,which did not affect the development of treatment,the remaining patients did not experience any adverse reactions.Conclusion:1.Chinese herbal thumb jars used for the treatment of AS patients with kidney deficiency and coldness can improve the symptom index(BASDAI,BASFI,TCM symptom score),sign index(thoracic mobility,occipital wall distance,Schober test)and laboratory index(CRP,ESR),and the comprehensive efficacy is better than that of glass fire jars and boiled thumb jars,which has clinical promotion significance.2.The use of Chinese herbal thumb jars for the treatment of patients with AS with kidney deficiency and coldness in the governor is highly operable,safe and worth promoting. |
