| ObjectiveThe increase in postoperative drainage in patients undergoing thyroid surgery may increase the likelihood of postoperative complications.At present,multiple factors are known to affect postoperative drainage of thyroid.This article intends to analyze the relationship between the patient’s gender,age,body mass index(BMI),serum albumin,hypertension,diabetes,time of vacuum drainage and catheterization and drainage and drainage time after thyroidectomy,and discuss unilateral total thyroidectomy Influencing factors of postoperative drainage.MethodsThe clinical data of 87 patients who underwent unilateral total thyroidectomy + central lymph node dissection who were admitted to the Department of Breast and Thyroid Surgery of Qinghai University Hospital from November 2019 to July 2020 were analyzed.SPSS25.0software was used for statistical analysis to analyze the relationship between the patient’s gender,age,body mass index(BMI),serum albumin,hypertension,diabetes,time of negative pressure drainage and catheterization after thyroid surgery,To explore the influencing factors of drainage after unilateral total thyroidectomy.Results(1)Male drainage at 0-12 h,12-24 h after surgery,24-36 h after surgery,and total drainage at extubation were significantly higher than females(P<0.05),gender is the influence An important factor behind the flow of drainage.(2)The young group(25 years old ≤ age <45 years old),middle-aged group(45 years old≤ age <60 years old)and the elderly group(60 years old ≤ age ≤ 79 years old)3 groups of patients on the second day after surgery,surgery There was a statistical difference between the total postoperative drainage volume,and age had a significant influence on the postoperative drainage volume(P<0.001).(3)Three groups of patients in the normal group(20kg/m2≦BMI<23kg/m2),overweight group(23kg/m2≦BMI≦25kg/m2)and obesity group(25kg/m2<BMI≦28kg/m2)after surgery There was no statistical difference between the drainage volume at 0-12 h postoperatively and the drainage volume at 12-24 h postoperatively(P>0.05).There was a statistical difference between the drainage volume at 24-36 h postoperatively(P=0.048)and the total drainage volume at the time of extubation.(P=0.025).BMI has a certain influence on the total drainage volume after surgery.(4)Low protein group(38g/L≦serum albumin<45g/L),normal protein group(45g/L≦serum albumin≦55g/L),high protein group(55g/L<serum albumin≦58g)/L)Statistically Significant difference between the three groups 0-12 h drainage after operation,postoperative 12-24 h drainage volume,postoperative 24-36 h drainage volume,and total drainage volume at the time of extubation(P<0.05),serum white Protein is an influencing factor of postoperative drainage.(5)Intubation time group A(3 days),intubation time group B(4 days),and intubation time group C(Intubation time ≥ 5 days).There is a significant difference in drainage between 3groups 0-12 h after operation 12-24 h postoperatively,24-36 h postoperative drainage volume,and total drainage volume upon extubation(P<0.001).The catheterization time was an important factor affecting postoperative drainage.(6)Whether the patient has lymph node metastasis in the central cervical area,which means no significant difference between 0-12 hours after surgery,12-24 hours after surgery,24-36 h a few hours after the operation,the total displacement is at the sterner operation(P>0.05).(7)Between patients with diabetes or with hypertension and patients without diabetes or hypertension,the amount of drainage between 0-12 h after operation,Displacement between postoperative 0-12 h after 12-24 h displacement,24-36 h after drainage,the total drainage no statistics on age at extubation the difference between the flow(P> 0.05).(8)There are 4 influencing factors for the total postoperative drainage volume: age,BMI,serum albumin and drainage tube intubation time;postoperative drainage volume at 0-12 h and albumin and catheterization time.(9)The catheterization time has a significant positive predictive effect on the drainage volume at 0-12 h,the drainage volume at 12-24 h after the surgery,in drainage 12-24 h,24-36 h after surgery drainage volume after surgery,and the total drainage volume at the time of extubation(P<0.001).ConclusionsThe gender,age,BMI,serum albumin,and drainage tube placement time of patients undergoing unilateral total thyroidectomy have an impact on the postoperative drainage of the patient.Whether it is complicated by hypertension,diabetes,and whether it is accompanied by central lymph node metastasis.There is no obvious impact on the amount of rear drainage.This study believes that doctors can be based on the relevant factors that affect negative pressure drainage in clinical work,such as age,BMI,preoperative serum albumin,and drainage tube placement time.Carry out preoperative assessment of patients,anticipate the possibility of increased drainage in advance,and carry out corresponding interventions,such as recommending that patients control their weight and blood pressure and blood sugar within the ideal range,so as to achieve the purpose of promoting the patient’s body to recover as soon as possible.Provide clinicians with new ideas to prevent and reduce complications after thyroid surgery. |
