Objective:Objective to compare the clinical efficacy of surgical treatment of early non-small cell lung cancer(NSCLC)and stereotactic body radiotherapy(SBRT)in the treatment of early NSCLC.Method:Methods: The clinical data of 43 patients with primary stage I and II NSCLC who received stereotactic radiotherapy and complete follow-up data in our hospital from October 2015 to October 2020 were retrospectively analyzed.The data of 43 patients who received SBRT were selected as the control group.According to gender,age and tumor type,300 patients with NSCLC who received single port video-assisted thoracoscopy(spvats)treatment in our hospital in the past three years were analyzed 41 patients were selected according to the four matching factors of location and tumor stage.The hospitalization time,the OS rate,the PFS rate in the first,second and third years,the quality of life,the changes in lung function,the incidence rate of respiratory complications and the incidence rate of cardiovascular complications of SBRT group and SPVATS group were compared between the two groups after surgery.Result:A total of 82 patients in the two groups were successfully treated,and no perioperative death and serious complications occurred.All patients received wechat follow-up and reexamination regularly.Among them,the follow-up time was 5-60 months,the median follow-up time was 32 months,and the follow-up rate was 96%.The average length of hospital stay was 8.1 days in spvats group and 3.2 days in SBRT group.The OS rates were 97.6%,87.8% and 75.6% in spvats group and 97.6%,85.4% and 70.7% in SBRT group respectively after 1,2 and 3 years of follow-up(P>0.05).The PFS rates were 87.8%,80.1% and 68.3% in spvats group and 90.2%,78.0% and 65.9% in SBRT group respectively.There was no significant difference in PFS between the two groups(P>0.05);the postoperative quality of life in the SBRT group was significantly better than that of the SPVATS group,which was statistically significant(P<0.05);the SBRT group had respiratory complications The incidence of disease and cardiovascular system complications was lower than that in the SPVATS group,and there was a statistical difference between the two groups(P<0.05).Conclusion:There was no significant difference in OS,PFS between spvats and SBRT in the treatment of early non-small cell lung cancer.However,SBRT group had the advantages of shorter hospital stay,less trauma or no trauma,more beauty,less pain and less adverse reactions.It also provided safety and safety for some patients who could not tolerate surgery Effective new method.
