| Objective:Chemotherapy-Induced nausea and vomiting is the most common adverse reaction of digestive tract in patients with malignant tumor during chemotherapy.This study evaluates the effectiveness of Wendan Decoction plus acupuncture for nausea and vomiting caused by platinum-based chemotherapy in patients with malignant tumors,giving full play to the advantages of traditional Chinese medicine,providing a new method to control nausea and vomiting after chemotherapy,and reducing the pain caused by chemotherapeutic drugs in the process of chemotherapy,improving curative effect,prolonging life,and providing clinical basis and reference for the promotion of acupuncture combined with medicine in the treatment of chemotherapy-related nausea and vomiting.Methods: In a randomized controlled study,64 patients with malignant tumors who were hospitalized in the Department of Hematology and Oncology,Qinghai Hospital of traditional Chinese Medicine from October 2019 to December 2020 were randomly separated two teams.In the control group,ondansetron hydrochloride 4mg was injected intravenously before and after chemotherapy,and dexamethasone 5mg was injected intravenously once before chemotherapy in the control group.On the basis of the antiemetic regimen in the control group,the treatment group was given oral granule(Wendan decoction)on the day of chemotherapy,once in the morning and evening,and acupuncture(Neiguan,Hegu,Zusanli)was given before chemotherapy,and the modus of leveling tonic and relieving was accepted,and the needle was retained for half an hour after obtaining qi for 6 days.According to the CTCAEv4.0 standard of the National Cancer Institute of the United States,the frequency and severity of nausea and vomiting in the whole course(d1-d6),acute phase(D1)and delayed phase(d2-d6)and the proportion of rescue drug use in the two groups were recorded to the time of treatment failure(from the beginning of chemotherapy to the first occurrence of vomiting or intervention with salvage drugs).The complete remission rate(no vomiting,no use of salvage drugs)and complete control rate(no vomiting,no use of salvage drugs,no more than mild nausea)were calculated.At the same time,adverse events during chemotherapy were recorded and statistically analyzed by SPSS17.0 software to evaluate the clinical efficacy and safety of the two groups.Results: 1.The baseline of sex,age,height,weight,tumor type,drinking history and chemotherapy history was consistent between the control group and the treatment group(P>0.05).2.The complete response rate of the control group(50%)was lower than that of the treatment group(62.5%)(P>0.05);the complete response rate of the control group in the acute phase was lower than that of the treatment group(62.5%VS71.9%)(P>0.05);The complete response rate in the delayed phase of the treatment group was higher than that of the control group by 56.3% VS 81.3%(P<0.05).3.The complete control rates of the control group and the treatment group in the whole course,acute stage and delayed stage were 21.9% VS50%(P<0.05),46.9%VS65.6%(P>0.05),43.8%VS75%(P<0.05),respectively.4.The control group and the treatment group failed 16 times and 12 times.The average TTF of the control group(102.4±9.7h)and the average TTF(76.8±11.9h)of the treatment group were P>0.05 after log-rank test,which was not statistically significant.5.There was no significant difference in the severity of nausea and vomiting between the control group and the treatment group in the acute phase(P>0.05).The degree of nausea and vomiting in the delayed phase of the treatment group was milder than that of the control group(P<0.05).6.No acupuncture and other adverse events such as needle fainting,broken needle,hematoma,etc.were found in thecontrol group.Conclusions:1.Modified Wendan decotion combined with acupuncture and routine western medicine antiemetic regimen can reduce the incidence of delayed nausea and vomiting caused by platinum-containing chemotherapy,but has no obvious effect on relieving acute nausea and vomiting.2.Modified Wendan decotion combined with acupuncture and routine antiemetic regimens of western medicine can reduce the severity of delayed nausea and vomiting caused by platinum-containing chemotherapy. |
