Abstract/Summary:
Objective:Discussion on the clinical significance and safety of ureteral branches of ureteral artery in cervical cancer.Methods:From April 2017 to December 2017 due to cervical cancer,a total of 99 cases were screened in all patients who were clintered in the Third Affiliated Hospital of Zunyi Medical University,99 cases were obtained,and 99 cases were obtained according to the treatment used in actual conditions.Solution,the root-cured uterus peese and pelvic lymph nodes and pelvic lymph nodes of uterus were treated with a total of 56 cases;43 cases of traditional RH and pelvic lymph nodes were controlled.Collecting its surgical data,video data,and conducts three-dimensional reconstruction,pathological information,study laparoscopic Doppler blood flow monitoring materials and other clinical data and postoperative follow-up data,all data statistical analysis use SPSS18.0 Software is done.Results:1.The study group and the control group were compared from the age,the body mass index,histocale,surgical duration,and the number of times were compared to the number of lymph nodes(p > 0.05).2.There were 43 cases in the control group,including 7 cases of ureteral stenosis,hydronephrosis,and 1 case of uretero-vaginal fistula;the study group had 56 cases,and none of the above-mentioned urinary complications occurred.The incidence of complications in the posterior urinary system is statistically significant(p = 0.001)。3.Through three-dimensional reconstruction(Three dimensions reconstructure,3D),all the ureter branches of the uterine artery seen before the operation were successfully preserved,and the diameter of the ureter branches of the uterine artery before the operation averaged(0.69±0.09)mm,and the ureter of the uterine artery after the operation The average branch diameter was(1.18±0.12)mm,and the difference was statistically significant(p < 0.001).The residual uterine artery blood flow index was measured during the operation,and the maximum systolic velocity(Peak systolic velocity,PSV),resistance index(RI)and pulse index(PI)of the systole were all Compared with the pre-cut,the difference was statistically significant(p <0.001).4.3 cases had lymph node metastasis to the uterine artery.The metastases of other pelvic tissues,lymph nodes and cervical canal were significantly correlated with the metastases of para-uterine artery lymph nodes(p < 0.05).Conclusion: 1.Radical hysterectomy that preserves the ureteral branch of the uterine artery does not affect the scope of surgical resection and can reduce the incidence of postoperative urinary complications.2.For tumor diameter less than 2 cm,the depth of infiltration is shallow,and there is no invasive vanity of the palace.RH surgery to retain the uterus artery uterus;for pelvic lymph nodes,the cervical is involved,the cervical cancer is involvement or has high risk factors in cervical cancer Patients do not recommend RH surgery for retaining uterus vessels,recommend traditional RH surgery.
