Objective:Compare the outcomes and side effects between the LISA group and the INSURE group in premature infants with respiratory distress syndrome(RDS).Method:This study included 56 children who met the inclusion criteria.In the experimental group,20 cases of PS administration completed by LISA(less invasive surfactant administration)technology were admitted to 8 neonatal intensive care units(NICUs)in Anhui Province from August 2019 to April 2020.In LISA group,six cases were provided by Anqing municipal hospital,four cases were provided by the first Hospital Affiliated of USTC,three cases were provided by Bozhou people’s hospital,two cases were provided by Fuyang people’s hospital,two cases were provided by Tongling people’s hospital,one case was provided by the third the people’s hospital of Bengbu,one case was provided by Huaibei maternity and child health care hospital,one case from The People’s Hospital of Chizhou.36 infants delivered surfactant by INSURE(intubate-surfactant-extubate)technique were served as a control group,who hospitalized in the first Hospital Affiliated of USTC from January 2017 to January2020.The basic information of children and mothers,arterial blood gas values before and after administration,oxygen saturation and heart rate at administration,complications and oxygen use during hospitalization were collected.The t test and chi-square test were used to compare and analyze the data of the two groups.Results :1.In terms of baseline characteristics :The basic information of children included gestational age(week),birth weight(g),gender,multiple births,test tube,apgar score≤ 3 of infants;And The basic information of mothers included age(year),gestational diabetes,premature rupture of membrane(PROM)≥ 18 h,placental / amniotic fluid abnormalities,prenatal body temperature ≥ 37.5 °C,prenatal application of magnesium sulfate,and prenatal injection stimulation,all the above information was not statistically significant between LISA group and INSURE group,and was comparable.2.In terms of arterial blood gas value : the PH value of LISA group after administration was significantly higher than that before administration,the difference was statistically significant(p < 0.001);The PCO2 value in the LISA group after administration was significantly lower than that before administration,and the difference was statistically significant(p = 0.005).PO2 changes in LISA group before and after administration were not statistically significant;The PH value,PCO2 value and PO2 value of INSURE group had no significant changes before and after administration.The proportion of PH within normal range in LISA group was significantly higher than that in INSURE group(p = 0.009).3.In terms of complications : LISA group and INSURE group LISA can reduce the incidence of BPD and noninvasive failure rate,the difference was not statistically significant;4.In terms of side effects :During the administration,one case of heart rate decrease and three cases of blood oxygen saturation decrease occurred in the LISA group.While zero case of heart rate decrease and blood oxygen saturation decrease occurred in the INSURE group.The decrease of blood oxygen saturation was statistically significant between the two groups(P = 0.041).Conclusion:LISA technology can be carried out in infants with spontaneous breathing,which can significantly improve alveolar ventilation without increasing the incidence of complications such as BPD.
