Objective: The purpose of this study was to explore the high expression of RIP1(receptor-interacting protein kinase 1),NOX4(NADPH oxidase 4),PARP1 [poly(ADP-ribose)polymerase-1] and AIF(apoptosis inducing factor)in vitreous after rhegmatogenous retinal detachment(RRD),and to explore the correlation between the expression level of proteins and macular involvement,the extent of retinal detachment and the duration of RD,so as to further prove the occurrence of necroptosis after retinal detachment in photoreceptors and provide a basis for further clinical drug intervention.Methods: Vitreous was collected from 40 patients undergoing vitrectomy,including 20 patients with primary RRD(rhegmatogenous retinal detachment)as RD group and 20 patients with idiopathic macular hole(IMH)or idiopathic anterior macular(IEM)patients as control group.The concentrations of RIP1,NOX4,PARP1 and AIF in vitreous were measured by ELISA.Record the patient’s age,sex,diagnosis,course of disease,preoperative / postoperative visual acuity,lens status,as well as the number and location of retinal holes,macular involvement and the extent of retinal detachment in patients with RD.The expression of necroptotic proteins in vitreous of patients in RD group and the control group was analyzed,and the correlation between the expression level of the protein in the RD group and clinical parameters such as macular involvement,the extent of RD and the duration of RD was analyzed.Results:1.There was no significant difference in sex and age between RD group and Control group(P > 0.05).2.The concentration of RIP1 in vitreous of RD patients was 33.43±2.35 ng/ml,the concentration of NOX4 was 36.05±4.56 μg/ml,the concentration of PARP1 was2867±215 pg/ml,the concentration of AIF was 8.97±1.12 ng/ml.In Control group,the concentration of RIP1 in vitreous was 21.15±3.39 ng/ml,the concentration of NOX4 was 28.17±4.53 μg/ml,the concentration of PARP1 was 2050±315pg/ml,the concentration of AIF was 4.65±1.22 ng/ml,respectively.There were significant differences in the concentrations of RIP1,NOX4,PARP1 and AIF between the two groups(p<0.05).3.The concentration of RIP1 in the vitreous of RD patients with macula on was31.14±1.22 ng/ml,the concentration of NOX4 was 30.87±1.50 μg/ml,the concentration of PARP1 was 2675±183.10 pg/ml,the concentration of AIF was 7.83±0.84 ng/ml;The concentration of RIP1 in the vitreous of RD patients with macula off was 34.04±2.21ng/ml,the concentration of NOX4 was 37.35±4.11 μg/ml,the concentration of PARP1 was 2915±198.80 pg/ml,the concentration of AIF was 9.25±1.01 ng/ml.The expression of four proteins in vitreous of patients with macular off in RD group was higher than that in RRD patients with macular on.There was significant difference in the expression of RIP1,NOX4,PARP1 and AIF between the two groups(P= 0.022,0.007,0.042,0.019,respectively).4.The average duration of disease in patients with RD was 25.25 day,ranging from 9-90 days.Pearson correlation analysis showed that there was no correlation between the concentration of RIP1 and the duration of RD(P=0.550),there was no correlation between the concentration of NOX4 and the duration of RD(P=0.147),and there was no correlation between the concentration of PARP1 and the duration of RD(P=0.596).There was a significant correlation between AIF and the duration of RD(R=0.322,P=0.009),and the concentration of AIF in vitreous increased significantly with the duration of RD.5.In the RD group,Spearman correlation test showed that there was no correlation between the concentration of RIP1 and the extent of RD(P=0.376),there was no correlation between the concentration of NOX4 and the extent of RD(P=0.169),and there was no correlation between the concentration of PARP1 and the extent of RD(P=0.886);the concentration of AIF had no correlation with the extent of RD(P=0.051).Conclusion:1.This study found for the first time that the high expression of necroptotic proteins such as RIP1,NOX4,PARP1 and AIF in vitreous of patients with RD,which confirmed the existence of necroptosis in photoreceptors after human retinal detachment and provided a basis for further clinical intervention.2.RIP1,NOX4,PARP1 and AIF may be related to RD with macula off.3.AIF is related to the duration of RD,which may be a good index to reflect the duration of RD.4.RIP1,NOX4,PARP1 and AIF may not be correlated with the extent of RD.
