Study On The Frailty Status And Influencing Factors In Maintenance Hemodialysis Patients

Abstract/Summary:
Objectives:To understand the current status of frailty among maintenance hemodialysis patients,in this study,we analyze the influencing factors of frailty.Moreover,related suggestions are put forward based on the specific conditions,aiming to provide a reference for the development of effective intervention strategies to delay the frailty of patients.Methods:Application of cross-sectional research,A convenience sample of maintenance hemodialysis patients was selected from the blood purification center of hospitals from May to October 2020.The general situation questionnaire,FRAIL scale,CCI,PSQI,ADL,SGA,HAD and SSRS were used for evaluation.The database was set up with Epi Data3.1,and then was analyzed by SPSS 23.0.Descriptive statistical analysis,Mann-Whitney u test,Kruskal-Wallis H test,Spearman rank correlation,and ordinal logistic regressions were utilized to explore the current status of frailty and influencing factors among maintenance hemodialysis patients.Results:A total of 468 questionnaires were distributed and 460 were effectively received,with an effective response rate of 98.29%.A statistical analysis of460 research subjects,the results were as follows:1.Among the 460 maintenance hemodialysis patients,146(31.74%)were not frailty,177(38.48%)were pre-frailty,and 137(29.78%)were frailty.The incidence of fatigue and resistance in the various indicators of frailty were higher,which were 48.91% and 37.83%.2.Single factor analysis results showed that there were statistically significant differences in gender,age,marital status,education level,work status,physical exercise,abdominal obesity,multi-drug,primary disease,other chronic diseases(diabetes,cardiovascular disease,cerebrovascular disease,chronic obstructive pulmonary disease and cancer),parathyroid hormone,hemoglobin,albumin,KT/V,the scores of different charlson comorbidity Index and whether there was depression are related to the frailty of maintenance hemodialysis patients(P<0.05).3.Correlation analysis results showed that there is no correlation between the illness and sleep latency,sleep duration,sleep disturbances,use of sleeping medication and daytime dysfunction(P>0.05),the total frailty score and other dimensions were positively correlated with the total scores of PSQI and each dimension(P < 0.05);The total frailty score and each dimension were positively correlated with the total scores of ADL and SGA and each dimension(P<0.05);and there were negative correlations between the total score of SSRS and each dimension(P<0.05).4.Multivariate analysis showed that there were independent influencing factors for frailty in gender,age,marital status,parathyroid hormone,hemoglobin,albumin,the scores of different charlson comorbidity Index,sleep quality,activity of daily living,subjective nutritional status,depression and social support(P<0.05).Conclusion:1.The incidence of frailty in maintenance hemodialysis patients was higher.Fatigue was the main manifestation of frailty.The results suggested that medical staff should attach great importance to the problem of fatigue of dialysis patients.2.Gender,age,marital status,parathyroid hormone,hemoglobin,albumin,the scores of different charlson comorbidity Index,sleep quality,activity of daily living,subjective nutritional status,depression and social support were the independent influencing factors of frailty among hemodialysis patients.The medical and health industry should pay attention to the above issues of patients and take effective measures to delay their frailty process and to improve the quality of life of patients.
