Observation On The Analgesic Effect Of Acupuncture At Lieque Point On Different Types Of Cervical Spondylosis

Objective:Through observing the difference of clinical curative effect of acupuncture on different syndrome types of cervical spondylosis,and analied the best type of indication of Lieque point for intervention of Cervical spondylosis,and provide a scientific basis for the clinical selection of acupuncture treatment of Cervical spondylosis.Methods:According to the purpose of this research and the standard of the subjects,a total of 90 Cervical spondylosis examinee who conform to the inclusion criteria were were collected,including 30 cases in each of the Wind-cold obstruction type,the Qi-stagnation and blood-stasis type,the Liver and kidney deficiency type groups.The subjects’ diagnostic criteria were drawn up in accordance with the Criteria for the Diagnosis and Treatment of TCM Diseases developed by the State Administration of Traditional Chinese Medicine,and the criteria for syndrome differentiation and classification of the Wind-cold obstruction type,the Qi-stagnation and blood-stasis type,and the liver and kidney deficiency type of Cervical spondylosis were formulated by referring to the Therapeutics of Acupuncture and Moxibustion published by China Traditional Medicine Press.All the three groups were all treated by acupuncture at Lieque point combined with exercise therapy.The specific treatment is as follows:The patients were seated,selected bilateral columns of Lieque point,and after the skin at the acupoint area was disinfected with 75% alcohol,the stainless steel filiform needle with a diameter of 0.25 mm and a body length of 40 mm was selected for acupuncture.The acupuncturist applied the method of along-against-channel reinforing and reducing on the bilateral Lieque points,that is the needle tip is flat punctured in the direction of the elbow joint,and performed twirling reinforcing-reducing method after Deqi.During the process of implementing acupuncture technique,the twisting angle should be controlled at about 180°,and the frequency should be controlled at about 60 revolutions per minute.At the same time,patients were instructed to move their head and neck slowly from left to right and from top to bottom,ranging from small to large.The operation time of acupuncture manipulation was 2 minutes,the needles were left for30 minutes,and the needles were executed once every 10 minutes.The needle was removed after 30 minute.Patients in each group were treated once.Visual analog scale(VAS)scores and current pain intensity Inventory(PPI)scores were used as the observed measures in this study.Data statistics are performed using SPSS 25.0 statistics software.The differences in VAS and PPI scores before and after treatment among the three groups and the score differences among the three groups were compared to analyze the differences in acupuncture analgesic efficacy among the three groups before and after treatment.An objective clinical evaluation of the treatment outcomes of patients with the three syndromes is preferred to select the best indication type of Lieque point for intervention of cervical spondylosis.Result:1.After statistical analysis,there was no significant difference among the three groups in terms of age,gender,course of disease,VAS score and PPI score before treatment(P>0.05),which suggested that the three groups of the patients are comparable.2.Comparison of the VAS and PPI scores of the three groups of patients before and after treatment: the VAS and PPI scores of the patients with the Wind-cold obstruction tape of Cervical spondylosis and the Qi-stagnation and blood-stasis tape of Cervical spondylosis were significantly different(P < 0.01,P < 0.05),which showed that Lieque point has good analgesic effect on the Wind-cold obstruction tape and the Qi-stagnation and blood-stasis tape of Cervical spondylosis;while there were no significant differences in VAS and PPI scores between the before and after treatment values of the Liver and kidney deficient deficiency tape of Cervical spondylosis(P > 0.05),which suggested that Lieque point has no obvious analgesic effect on the Liver and kidney deficiency tape of Cervical spondylosis.3.Comparison of the difference in VAS and PPI scores before and after treatment among the three groups: the difference in VAS and PPI scores before and after treatment between the the Wind-cold obstruction tape and the Qi-stagnation and blood-stasis tape and the Liver and kidney deficiency tape of Cervical spondylosis was significant(P < 0.05,P <0.01),suggesting that the analgesic effect of the Wind-cold obstruction tape of Cervical spondylosis was better than that of the Qi-stagnation and blood-stasis tape and the Liver and kidney deficiency tape of Cervical spondylosis;The difference in VAS and PPI scores before and after treatment in the Qi-stagnation and blood-stasis tape of Cervical spondylosis was significantly different from that in the Liver and kidney deficiency tape of Cervical spondylosis(P < 0.05),suggesting that the analgesic effect of the Qi-stagnation blood stasis tape of Cervical spondylosis was better than that of the liver and kidney deficiency tape of Cervical spondylosis.4.The total effective rate was 96.67% in the Wind-cold obstruction group,60.00% in the Qi-stagnation and blood-stasis group,6.67% in the liver and kidney deficiency group.The total effective rate in the Wind-cold obstruction group was higher than that in the Qi-stagnation and blood-stasis group and the liver and kidney deficiency group,conclusion the clinical effect of the Wind-cold obstruction group is better than that of the Qi-stagnation and blood-stasis group and the liver and kidney deficiency group,and that of the the Qi-stagnation and blood-stasis group is better than that of the liver and kidney deficiency group.Conclusion:The best indication type of biasis in the intervention term of acupuncture deficiency acupoints is the Wind-cold obstruction type,which provides beneficial guidance for the clinical application of "Lieque(LU 7)for the disorders of the head and neck".
