Objective To study the clinical efficacy of antibacterial peptide RISE-AP12 gel in the treatment of moderate and severe chronic periodontitis. Methods 60 patients with moderate and severe chronic periodontitis were randomly divided into experimental group A(periodontal basic treatment + antimicrobial peptide rise-ap12),experimental group B(periodontal basic treatment + minocycline hydrochloride)and control group C(periodontal basic treatment),20 cases in each group.Patients in group A and group B were treated with antibacterial peptide RISE-AP12 gel and Minocycline Hydrochloride Ointment adjuvant therapy after 1weeks / week, respectively. After 4 weeks of treatment, patients in group C were treated only with periodontal foundation without medication. The bleeding index, probing depth and attachment loss of each group were observed at the first week and the fifth week of follow-up,and the changes of spirochete, cocci and bacillus in subgingival plaque were recorded from the first to the fifth week. Results 1.Compared with baseline(the first week)after treatment(the fifth week),the proportion of spirochete in three groups decreased significantly(group a > group B > group C,P < 0.05);The proportion of cocci and rectobacteria in group A was higher than that in group B and group C(P < 0.05).2.After one month of treatment, the degree of gingival redness, swelling and hyperplasia of patients in the adjuvant use of antimicrobial peptide rise-ap12 group was better than that in the minocycline hydrochloride group and the simple periodontal basic treatment group.3.After treatment, the bleeding index(BI),probing depth(PD)and attachment loss(AL)of the three groups decreased compared with their respective baseline, and the differences were statistically significant(P < 0.01).The treatment of the three groups was effective.4.Compared with baseline, the improvement range of bleeding index, probing depth and attachment loss in group a > group B > group C.LSD method was used to compare the three groups, and the differences were statistically significant(P < 0.05).5.There were no adverse reactions such as bleeding aggravation, gingival redness aggravation and periodontal pocket deepening in the use of antibacterial peptide rise-ap12 and minocycline hydrochloride group during the treatment. Conclusion 1.As an adjunctive drug in the treatment of moderate and severe chronic periodontitis, antibacterial peptide RISE-AP12 and minocycline hydrochloride have good bactericidal effect on spirochetes and other pathogenic bacteria, and the antibacterial peptide RISE-AP12 has higher bactericidal efficiency and better effect.2.The combination of antibacterial peptide RISE-AP12 and minocycline hydrochloride in periodontal basic treatment is helpful to the rapid regression of gingival inflammation, and the effect of antibacterial peptide RISE-AP12 is better.3.The antibacterial peptide RISE-AP12 is superior to minocycline hydrochloride in improving the bleeding index, depth of detection and loss of attachment in patients with moderate and severe chronic periodontitis.
