| Objectives1.To construct a scheme for preventing against perioperative pressure injury in patients with hip fracture surgery.2.To assess the effects from the application of the scheme in patients with hip fracture surgery.Method1.Scheme construction:According to the literature review,combined with the actual needs of patients with hip fracture,patients with the prevention of hip fracture surgery were initially formed.Experts from relevant fields were invited to conduct two rounds of letter inquiry to the preliminary plan and revised according to expert opinions to form the final plan for the prevention of hip fracture surgery patients against peri operative pressure injury.2.Intervention:From May 2020 to January 2021,40 patients with hip arthroplasty in a Grade 3A hospital in Fujian were selected,Counting patients from 1~40 according to the procedure,Computer-generated random numbers,In size,The first 20 cases were control group,The latter 20 cases were intervention group.72 patients with proximal femoral intramedullary needle fixation,From 1~72 cases,Based on computer-generated random numbers,In size,The first 36 cases were assigned to the control group,The latter 36 cases were assigned to the intervention group.Control group carried out nursing routine of PPI prevention.The intervention group carried out the plan of preventing perioperative stress injury.The demographic data of the two groups were recorded,the knowledge level of stress injury was understood,and the data of participation attitude,behavior,comfort of posture placement,incidence of hypothermia during operation,incidence of stress injury and.IBM SPSS Statistics V21.0 software was used to analyze and compare the differences between the two groups,and to evaluate the application effect of hip fracture patients participating in the prevention of perioperative stress injury.Results1.Formation of the scheme to prevent against perioperative pressure injuries in the patients with hip fracture surgery and a scheme-based guidebook for patientsThe scheme was constructed to help patients with hip fracture surgery prevent against perioperative pressure injury.The scheme consists of three primary entries,namely,organizational structure,patients’ participation and quality control.Based on the participation plan,and taking into account the results from expert consultation,result,a guidebook against PPI was prepared for the patients with hip fracture surgery.2.Evaluation of the effects of the scheme on preventing against perioperative pressure injuryThis study covered a total of 110 patients undergoing hip fracture surgery.Among them,40 patients underwent hip arthroplasty,and 70 patients with the proximal femoral nail internal fixation.There were 20 patients with hip arthroplasty and 35 patients with proximal femoral anti-rotation intramedullary needle fixation,in the control and intervention groups,respectively.A comparison of the differences between the groups(P>0.05)in BIM,education,hospital stay,age,place of residence and sex was not statistically significant.The two groups did not significantly differ in their basic information(P>0.05);The incidence of pressure injury,hypothermia 30 minutes after anesthesia and hypothermia 1 h after anesthesia in the two groups were significantly lower than those in the control group.The difference was statistically significant(P<0.05).There was no significant difference in the incidence of hypothermia(P>0.05);the incidence of hypothermia in 2 hours after anesthesia was not complete because of the different operation time,the data could not be analyzed statistically;the control group was lower than that in the intervention group(P>0.05).Conclusion1.For patients with hip fracture,it is scientific,reliable and feasible to participate in PPI prevention.2.The PPI prevention program can effectively prevent the occurrence of PPI and low body temperature,improve the comfort,knowledge level and satisfaction of patients with hip fracture surgery,and provide a new nursing mode for the prevention of PPI. |
