| ObjectiveMultiple system atrophy(MSA)has complex clinical features.Many clinical manifestations are similar to Parkinson’s disease(PD).It is difficult to distinguish between the two diseases.Magnetic resonance imaging(MRI)technology may provide a useful assessment for the differential diagnosis of these two diseases.This topic analyzes the special magnetic resonance parameters of MSA and PD patients based on the ZOOMit technology to explore its potential application value in the differential diagnosis of MSA and PD.MethodThis study adopted a retrospective research method and included 49 patients with MSA,PD and HC who met the diagnostic criteria.All enrolled patients completed MRI examination.Acquire the ZOOMit-DWI image,use the DKI model,outline the ROI area of the bilateral red nucleus and substantia nigra in the Image J software,extract the feature data of the ROI,obtain the MK and MD diffusion parameters through calculation.The mean values of MD and MK parameters were standardized for comparison between groups.Statistical method:counting data is expressed as rate(%),measurement data is expressed as(mean±SD),and Mann-Whitney U test(uneven variance)is used.Result1.A total of 49 patients were enrolled,of which 15 were in the multiple system atrophy group(MSA,including 8 patients with MSA-C and 7 patients with MSA-P),21 patients with Parkinson’s disease(PD)and 13 healthy controls(HC).Among them,women accounted for 39% and men accounted for 61%,with an overall average age of 61.98±9.06 years old.2.In the substantia nigra or red nucleus,compared with the HC,the MSA and its subgroup MSA-C had higher mean MK parameters(P<0.05).The mean MK parameter of the MSA-C was the highest among the four groups,and the difference was statistically significant compared with the PD(P<0.05);the mean value of MK parameters of the PD was higher than that of the HC,and there was also statistically significant(P<0.05).The mean value of MK parameters of the MSA-P is higher than that of the MSA-P,but it was not statistically significant(P>0.05);the mean value of the MK parameters of the MSA-C is greater than that of the MSA-P,and there it was statistically significant(P<0.05).However,the comparison of the mean values of MD parameters among the groups was not statistically significant(P>0.05).3.According to the ROC analysis of the MK parameters between the groups,it was found that the ROC curve between the HC,the MSA and the PD showed that the specificity of the HC and the MSA in the substantia nigra was lower(66.7%),and the other sensitivity and specificity are all above 70%.While the sensitivity between the MSA-Cvs MSA-P was lower(62.5%),and MSA-Cvs PD had low specificity(57.1%)in the red nucleus.But the others have high sensitivity and specificity in the substantia nigra or red nucleus.The AUC among the groups was distributed between 0.733-0.912.ConclusionBased on the ZOOMit technology,the MK parameters calculated by the DKI model can initially distinguish MSA and PD from HC,also can distinguish MSA-C from PD and MSA-P.It indicates that ZOOMit technology may have potential application value in the differential diagnosis of MSA and PD. |
