Objective To explore the effects of ketorolac tromethamine combined with dorsal penile nerve block on restlessness in recovery period of children with concealed penis.MethodsFrom January 2018 to May 2020,66 children with concealed penis operation under general anesthesia of tracheal intubation were selected in our hospital.The age was 6-12 years old,the weight was 25-45 kg,ASA was grade I.The children were randomly divided into experimental group(Group T)and control group(group C),33 cases in each group.TIVA was used in the two groups.Ten minutes before anesthesia induction,Ketorolac tromethamine 0.5 mg/kg was injected intravenously in group T and ketorolac tromethamine was not injected intravenously in group C.surgeons in both groups used 0.375% ropivacaine 3 ml for dorsal penile nerve block before operation.1.The age,weight and operation time were compared between the two groups,the amount of bleeding,infusion and urine were compared between the two groups.The dosage of propofol and remifentanil and the extubation time were compared between the two groups.2.The scores of restlessness,the incidence of restlessness and the score of FLACC at 30 minutes after extubation were compared between the two groups.The other adverse events were compared between the two groups.Results66 children,aged from 6 to 12 years old,weighing 25-45 kg,were enrolled in this study.The age of the two groups were(8.33±1.86)vs(8.40±1.61)year old.The weight of the two groups were(31.90±3.44)vs(31.27±3.17)kg.The operation time of the two groups were(59.57±4.39)vs(59.36±4.53)min.The amount of bleeding of the two groups were(12.83±2.55)vs(12.47±2.57)ml.The infusion volume of the two groups were(233.26±6.70)vs(230.80±7.85)ml.The urine output of the two groups were(54.63±2.03)vs(53.63±1.92)ml.In the general information mentioned above,there was no significant difference between the two groups(P>0.05).1.The dosage of propofol in group T were significantly lower than group C(189.33±45.06 vs225.37±27.36,P<0.05).The dosage of remifentanil in group T were significantly lower than group C(215.83±58.20 vs 255.20±58.81,P<0.05),Tht extubation time in group T were significantly lower than group C(16.03±2.66 vs 18.97±3.81,P<0.05).2.The scores of restlessness in group T were significantly lower than group C[2.00(1.00,2.00)vs 3.00(2.00,3.00),P<0.05].The restlessness rate in group T were significantly lower than group C(16.7% vs 56.67%,P<0.05).The FLACC at 30 minutes after extubation in group T were significantly lower than group C[3.00(2.00,4.00)vs 4.00(3.00,4.25),P<0.05].There was no significant difference in adverse reactions between the two groups 24 hours after operation(P>0.05).ConclusionKetorolac tromethamine combined with dorsal penile nerve block can reduce the incidence of restlessness,restlessness score and pain score,reduce the dosage of propofol and remifentanil,and shorten the time of extubation.The effect is better than that of dorsal penile nerve block alone.It has better safety.
