Analysis Of Influencing Factors Of Thyroid Cancer’s Recurrence In Female Breast Cancer Patients

Objective:Through observing the clinical characteristics of patients with recurrence of thyroid cancer after breast cancer surgery,and analyzing the related factors that may affect the recurrence of thyroid cancer in breast cancer patients,in order to discover the risk factors that may affect breast cancer patients’ recurrence of thyroid cancer after surgery.Methods:In the First Affiliated Hospital of Shihezi University,we retrieved the electronic medical record query system and the paper version of the archived medical records,and collected the case data of breast cancer patients who were admitted to the hospital from January 2014 to June 2020.A total of 716 cases met the inclusion and exclusion criteria.Among them,18 cases recurred thyroid cancer after breast cancer surgery.We set the 18 patients as the case group(group A),and randomly select 50 cases from the remaining 698 cases as control group(group B),then collected the age,height,weight(calculate the body mass index(BMI)),pathological examination results,number of births,immunohistochemical indexes,carcinoembryonic antigen,blood lipid indexes and radiation treatment status.Then we analyze and compare the differences between the two groups by statistical methods to draw conclusions.Results:We included 68 patients in total,including 18 cases of recurrent thyroid cancer after breast cancer surgery,and 50 cases of pure breast cancer.The average age of patients in the case group was(49.67±6.65)years old,and the average age of patients in the control group was(54.70±9.95)years old.The average body mass index(BMI)of the case group was(24.19±2.91)kg/m2;the average BMI of the control group was(24.31±3.02)kg/m2.There are 9 cases in the case group with tumor diameter ≤ 2 cm and 21 cases in the control group;7 cases in the case group with tumor diameters greater than 2 cm and ≤ 5 cm,19 cases in the control group;2 cases in the case group with tumor diameters> 5 cm,10 cases in the control group.There are 5 cases in the case group with lymph node metastasis and 14 cases in the control group.There is 1 case in the case group with distant metastasis and 2 cases in the control group.There are 15 cases in the case group suffered from invasive breast cancer,42 cases in the control group,3 cases in the case group suffered from non-invasive breast cancer,6 cases in the control group,0 case in the case group suffered from other types of breast cancer,and 2 cases in the control group.There is 1 case in the case group of patients with 0 births and 2 in the control group;15cases in the case group of patients with 1 to 2 births(including 2)and 40 cases in the control group;as for the number of births greater than 2,there are 2 patients in the case group and 8patients in the control group.There were 0 cases in the case group of patients with elevated carcinoembryonic antigen(CEA),6 cases in the control group,18 cases in the case group of patients with normal CEA,and 44 cases in the control group.There were 3 cases in the case group of patients with elevated triglycerides,11 cases in the control group,15 cases in the case group of patients with normal triglycerides,and 39 cases in the control group.There were 3 cases in the case group of patients with elevated total cholesterol,17 cases in the control group,15 cases in the case group of patients with normal total cholesterol,and 33 cases in the control group.There are 2 cases in the case group of patients with elevated low-density lipoprotein,6 cases in the control group,16 cases in the case group of patients with normal low-density lipoprotein,and 44 cases in the control group.There are 2 cases in the case group of patients with elevated high-density lipoprotein,18 cases in the control group,16 cases in the case group of patients with normal high-density lipoprotein,and 32 cases in the control group.There are 15 cases in the case group of estrogen receptor-positive patients,21 cases in the control group,3 cases in the case group of estrogen receptor-negative patients,and 29 cases in the control group.There were 16 cases of progesterone receptor-positive patients in the case group,27 cases in the control group,2 cases of progesterone receptor-negative patients in the case group,and 23 cases in the control group.There are 3 cases of human epidermal growth factor receptor 2(Her-2)positive patients in the case group,8 cases in the control group,15 cases of Her-2 negative patients in the case group,and 42 cases in the control group.There are 5 cases of patients receiving radiotherapy after breast cancer surgery in the case group,and 4 cases in the control group.Comparing the positive rate of estrogen receptor,the positive rate of progesterone receptor,and the rate of postoperative radiotherapy between the two groups,the observed values in the case group were higher than those in the control group,P <0.05,the differences between the two groups were statistically significant.Tumor size and pathological type,lymph node metastasis,distant metastasis,number of births,serum carcinoembryonic antigen,triglycerides,total cholesterol,low density lipoprotein,high density lipoprotein and positive rate of Her-2 were not significantly different between the two groups,P >0.05.The results of multivariate logistic regression analysis showed that positive expression of ER[OR(95%CI):8.023(1.871-34.405)],positive expression of PR[OR(95%CI):7.603(1.469-39.358)],postoperative radiotherapy[OR(95%CI):5.853(1.084-31.596)] are independent risk factors for the recurrence of thyroid cancer after breast cancer surgery.Conclusion:Positive expression of estrogen receptor,positive expression of progesterone receptor and radiotherapy after breast cancer surgery may increase the risk of recurrence of thyroid cancer after breast cancer surgery,and they are independent risk factors for recurring thyroid cancer after breast cancer surgery.
