Objective:To investigate the relationships between carotid-femoral pulse wave velocity(cfPWV)and atherogenic index of plasma(AIP),atherosclerosis index(AI)in adults.Methods:This study selected data from January 2016 to January 2020 were selected from Fuzhou research database for analysis.Fuzhou Study(registration number:ChiCTR2000039448)is a non-randomized,single-blind,observational study investigating hypertension target organ damage and risk factors based on outpatients and inpatients in the general practice and cadres ward of the First Affiliated Hospital of Fujian Medical University.Subjects were included,and age,gender,history of hypertension and other related medical history,status of medical treatment received,height,weight and waist circumference(WC),blood pressure,heart rate of all subjects were recorded,and cfPWV is measured with Complior Analyse.Blood urea nitrogen(BUN),serum creatinine(Scr),uric acid(UA),fasting blood glucose(FBG),cystatin C(Cys C),homocysteine(Hcy),glycosylated hemoglobin Alc(HbAlc),total cholesterol(TC),triglyceride(TG)and high density lipoprotein cholesterol(HDL-C)were detenmined by the same automatic biochemical analyzer P800.AIP and AI were calculated by the following formula,AIP=Log(TC/HDLC),AI=(TC-HDL-C)/HDL-C.A total of 3470 cases were included in the database,and 1398 cases of observed objects meeting the criteria were included.According to the European association of hypertension on using carotid artery pulse wave conduction velocity for detection of arterial stiffness expert consensus,the cfPWV cut-off point for predicting cardiovascular events is 10m/s,all subj ects were divided into cfPWV normal group and elevated cfPWV groupbased on the cut-point of cfPWV at 10m/s.The correlation analysis,multiple linear regression and Logistic regression to explore relationships between cfPWV and AIP,AI.ROC curve analysis is performed to calculate the best predictive value of AIP and AI,as well as their sensitivity and specificity.Finally,all subjects were stratified according to the prevalence of hypertension,gender and age,and the dichotomous Logistic regression is performed to analyse the relationship between cfPWV and AIP,AI.Result:1.Compared with the normal cfPWV group,the percentage of hypertension patients and men in the elevated cfPWV group is higher(P<0.05),the levels of AIP,AI,age,BMI,WC,SBP,DBP,HR,BUN,Scr,UA,FBG,Cys C,Hcy and HbAlc are significantly increased(P<0.05),and the level of eGFR is significantly decreased(P<0.05).2.In all subjects,the Spearman correlation analysis was performed,mild to moderate correlations were found as follows:AIP,AI,prevalence of hypertension,male,age,WC,SBP,DBP,HR,UA,FBG,Cys C,Hey,HbAlc are positively correlated with cfPWV(P<0.05),while eGFR is negatively correlated with cfPWV(P<0.05).The stepwise multiple linear regression is further performed,AIP and AI are independent related factors of cfPWV(β=0.156,0.070,P<0.05).In addition,age,SBP,HR,UA,Hcy and HbAlc are also independent related factors of cfPWV.3.In all subjects,the logistic regression showed that the incidence of cfPWV≥10m/s,increasing along with the increase of AIP and AI(OR=18.291、1.568,P<0.05),suggesting that the AIP and AI are an influencing factor of arteriosclerosis.After adjusting the influence of cardiovascular risk factors such as hypertension and age,the above conclusions are still valid.The ROC curve analysis is performed with cfPWV as the diagonal line.The areas under the ROC curve of AIP and AI are 0.697 and 0.627 respectively.The critical points of AIP and AI were determined as 0.00 and 3.02 by the Youden index.When AIP>0.00 and AI>3.02,it suggested that there might be a trend of increasing cfPWV(sensitivity of the former is 76.2%,specificity is 54.3%.sensitivity of the latter is 56.3%,specificity is 64.0%).4.After stratification by hypertension,sex and age,stepwise multiple linear regression showed age and blood pressure were major independent related factors of cfPWV.In the young and middle-aged group with normal cfPWV,AIP is an independent related factor of cfPWV(β=0.067,P<0.05).In the normal blood pressure group with normal cfPWV,AI is an independent related factor of cfPWV(β=0.097,P<0.05).Conclusion:1.In adults,in addition to age,SBP,HR,UA,Hcy and HbAlc,AIP and AI were also independent related factors for the increase of cfPWV.Among them,age and blood pressure had the greatest effect on the increase of cfPWV.2.In adults,the increase of AIP and AI is the influencing factor of cfPWV≥10m/s,namely,AIP and AI are the influencing factors of arteriosclerosis.When AIP>0.00 and AI>3.02,they have certain predictive value in the screening of arteriosclerosis. |
