Objective:We aimed to introduce the clinical features and analyze prognostic factors of COVID-19.Methods:Data of 113 cases of COVID-19 patients in Hubei Hospital of traditional Chinese Medicine were retrospectively collected and analyzed from January 1,2020 to February 7,2020.The cases data of gender,age,underlying diseases and so on were collected.Clinical classification was performed according to the 5th edition of national recommendations for diagnosis and treatment of respiratory infections caused by2019-n Co V.Independent sample t test,Mann Whitney U test,Chi-squared test and Fisher’s precise test were used to assess between-group differences.The Kaplan-Meier method was used to analyze single factor which may influence the prognosis.Multivariate Logistic regression analysis was utilized to analyze independent risk factors that might influence the prognosis of COVID-19.Results:Of the 113 cases,73 moderate cases and 40 severe cases.The average age of the patients was 51.39±16.54 years,and the median hospital stay was 16 days.The common clinical manifestations were fever(102/113,90.27%),cough(48/113,42.48%)and fatigue(24/113,21.24%).88.29%(98/111)of the patients presented with lymphocytopenia,which was more common in the severe case group(97.50%vs 83.10%,P=0.050).The levels of alanine aminotransferase(32.18±24.30U/L vs 23.42±17.77U/L,P=0.012),aspartate aminotransferase(35.90±16.80U/L vs 25.42±19.14U/L,P<0.001),γ-glutamine transaminase(32.68±25.68U/L vs 25.36±27.76U/L,P=0.024),total bilirubin(10.46±8.17umol/L vs 7.22±2.78umol/L,P=0.008),C-reactive protein(52.03±39.27mg/L vs 26.59±31.82mg/L,P<0.001),lactate dehydrogenase(272.41±82.14U/L vs194.87±65.22U/L,P<0.001)in severe patients were all higher and the level of albumin(36.15±4.19g/L vs 38.04±3.80g/L,P=0.014)was lower than those in moderate one,the difference was statistically significant.Among them,93.81%(106/113)patients received anti-virus therapy,61.06%(69/113)received gamma globulin,62.83%(71/113)received glucocorticoid and 94.69%(107/113)received traditional Chinese medicine.The Clinical classification(P=0.002),age(P=0.001),and sex(P=0.031)showed by The Kaplan-Meier method had an impact on the prognosis of COVID-19.while basic diseases(P=0.225)and lymphocytosis(P=0.267)had no effect.Multivariate analysis showed treatments such as glucocorticoid(P=0.125),antiviral drugs(P=0.973)and gamma globulin(P=0.413)cannot change the prognosis alone,where some patients may benefit from traditional Chinese medicine.Conclusions:Based on the data of this study,we found that COVID-19 is a serious infectious disease.Different clinical types directly affect the prognosis of patients,and the outcome of moderate type is better than that of severe one.Age(under 55 years old)and gender(female)may be the influencing factors to predict the prognosis of COVID-19.So far,there is no effective drugs,but comprehensive treatment including traditional Chinese medicine can improve the prognosis of COVID-19,which needs further exploration.
