| Objective: This study was to investigate the risk factors of parenteral nutrition associated cholestasis(PNAC)in premature neonates and to provide theoretical basis for clinical intervention.Methods: A total of 329 cases met the criteria and were targeted.They lived in the Neonate Department of the Second Hospital of Hebei Medical University from January 2018 to December 2020.The criteria is the parenteral nutrition(PN)duration ≥14 days,gestational age <37 weeks.The general clinical data and test results of liver function were statistically analyzed.Of the 329 subjects,PNAC group had cholestasis and the non-PNAC group had no cholestasis.PNAC group and non-PNAC group were studied in case-control study.Results:1.In the 329 cases,28 cases had cholestasis.The incidence of PNAC was 8.5%.The gestational age and birth weight in the PNAC group were significantly smaller than the non-PNAC group;2.The incidence of PNAC in the premature neonates with septicemia,bronchopulmonary dysplasia(BPD),retinopathy of prematurity(ROP),anemia have significantly increased;3.Total parenteral nutrition(TPN)duration,PN duration,time to start gastrointestinal feeding and average glucose consumption in the PNAC group were larger than non-PNAC group.Average amount of lipids was lower than that of non-PNAC group;4.Apgar 1 minute score,Apgar 5 minute score in the PNAC group had lower than the non-PNAC group.The use time of invasive ventilator and Non-invasive ventilator was longer in PNAC group than in non-PNAC group.Logistic regression showed PN duration >28 days,TPN duration >3 days,sepsis were independent risk factors for PNAC occurrence.Conclusion:1.The incidence of premature of PNAC in our hospital is 8.5% in three years.2.PN duration >28 days,TPN duration >3 days and sepsis were independent risk factors for PNAC. |
