Objective To explore the application value of dermoscopic image analysis technology in dynamic monitoring of 5-aminolevulinic acid photodynamic(ALA-PDT)in the treatment of actinic keratosis(AK)and follow-up.Methods Selected 80 patients who were diagnosed as facial actinic keratosis by skin histopathology from October 2018 to February 2020 at the Dermatology Clinic of the General Hospital of Ning xia Medical University as the research subjects,and were treated with ALA-PDT.Application Dermoscopy dynamic monitoring observes the dermoscopic image characteristics of actinic keratosis: red false mesh pattern,yellow and white scales,target-like hair follicles and rose petals changes,and the dermoscopic image characteristics are regarded as the end point of the treatment.Dermoscopy was used for a 12-month follow-up to observe the recurrence and new occurrence.Results A total of 75 AK patients completed the study,and a total of 164 facial AK skin lesions.When the AK skin lesions reached clinically assessed complete remission,a small number of dermatoscope image characteristics still persisted;The dermoscopic image features before treatment were 149(90.9%)red false mesh patterns,130(79.3%)yellow and white scales,93(56.7%)target-like hair follicles,and 71(43.3%)rose petal signs.After treatment Dermoscopic image features 56(34.1%)red false mesh patterns,14 yellow and white scales(8.5%),44 target-like hair follicles(26.8%),40 rose petal signs(24.4%),AK before and after treatment The difference of image characteristics under dermoscopy was statistically significant(P<0.05).For AK skin lesions with persistent dermoscopic image characteristics,continue to give 1-2 ALA-PDT treatment until the end point of the microscopic image characteristics of yellow and white scales,red false mesh pattern,target-like hair follicles and rose petals disappear completely.However,the rose petal sign of20 AK skin lesions was reduced compared with before,and it did not completely disappear.No treatment was given.All cases entered the follow-up period.After 12 months of follow-up with dermatoscope,no recurrence of AK was found;the original 20 AK skin lesions with the rose petal sign were observed gradually and completely disappeared on their own during the follow-up;9 cases of AK patients with the original AK skin lesions were observed at the same time In addition,new AK dermatoscope image features appeared successively in other parts,which respectively improved the pathological biopsy of skin tissues,and all indicated actinic keratosis.Conclusion 1.Dermoscopic image analysis technology can be used as a non-invasive,simple and easy-to-use better technology for the treatment of dynamic monitoring of AK.2.Dermoscopic image features: red false mesh pattern,yellow and white scales and target-like hair follicles can be used as the end point of treatment for AK to evaluate the curative effect;while the rose petal sign is only a reference value and should not be used as the end point of treatment.3.Dermoscopy image analysis technology is applied to the follow-up of AK,which can monitor the recurrence of AK,detect new AK skin lesions in time,and implement long-term health management for AK patients. |
