| Objiective:A cross-sectional survey was conducted to evaluate the level of fear of disease progression in patients after bladder cancer surgery,and to analyze the influencing facors of fear of disease progression;according to Lee-Jones′fear of cancer recurrence theoretical framework and using path analysis methods,we analyze the effect among illness perception,willingness to review compliance,social support and fear of progression.Methods:A continuous sampling method was used to investigate 204 patients after bladder cancer surgery in the Department of Urology in Shenyang,five measurements including the simplified questionnaire of fear of progression,the general information questionnaire,the brief illness perception questionnaire,perceived social support sale and cancer patients′willingness to review compliance questionnaire were taken in the study.Using SPSS25 software for data analysis,frequency and percentage was used to describe the demographics and disease-related data;median and interquartile range was used to describe the status of fear of disease progression;using rank-transformed nonparametric test,single factor analysis and multiple linear regression to analyze the factors of fear of disease progression;Sperman correlation method was used to analyze the relationship among fesr of disease progression,illness perception,willingness to review compliance and social support in patients with bladder cancer after surgery.The collected data above was also used for path analysis.Results:(1)The average score of fear of progression was(23.04±6.28)points and 8.8% of the patients(18 cases)scored ≥34 points,representing psychological dysfunction.(2)The univariate analysis showed that age,surgical options,time since diagnosis and recurrence times were statistically significance differences in the level of fear of progression(P<0.05).(3)Fear of progression was significantly positively correlated with illness perception(r=0.584,P<0.001),significantly negatively correlated with the dimension of social support-family members(r= - 0.255,P<0.001)and weak negatively correlated with willingness to review compliance(r=-0.146,P<0.05).(4)In the multivariate analysis of fear of disease progression in patients with bladder cancer after surgery,the variables entering the regression equation were illness perception,family dimension of social support,personal monthly income and age(P<0.05).(5)The direct effect value of “social support ” on “illness perception ” is-0.236,and the direct value “illness perception” on “fear of progression” is 0.640. “willingness to review compliance” and “social support ” had no significance on “fear of progression”.Conclusions:(1)The level of fear of progression and incidence of psychological dysfunction in bladder cancer patients after surgery were low.(2)Illness perception and family support were important factors.Medical staff should help patients establish correct disease cognition and reduce disease uncertainty,they also should encourage family members together with patients to participate in treatment decision-making and postoperative recovery.They were taught to listen to each other and express their concerns and thoughts,and guided to adopt a positive way to deal with it. |
