| Objective: To compare the clinical effect of single-layer and conventional three-layer in retroperitoneal laparoscopic adrenalectomy, and to study the application value of single-layer method in retroperitoneal laparoscopic adrenalectomy.Materials and methods: The research object of this subject is 80 patients with adrenal tumors who meet the inclusion criteria and were admitted by Department of Urology,Shengjing Hospital of China Medical University, from January 2016 to December 2020.Single-level method group(A group)36 cases,three-level method group(B group)46 cases, respectively compare the corresponding operation time, total postoperative drainage volume, postoperative drainage tube carrying time, postoperative hospital stay and other indicatorsResults: All the 80 cases were successfully completed, and none of them were entered ICU for treatment after operation. All patients recovered well after surgery and were discharged from hospital. There were 34 cases in group A,19 cases in partial resection,15 cases in complete resection;46 cases in group B,17 cases in partial resection,29 cases in complete resection. The diameter of adrenal tumor in the two groups were 1.92±0.70 cm and2.24±0.72 cm,respectively, there was no difference statistically significant(P>0.05).The operative time of the two groups were 81.56±22.60 min and 90.61±17.63 min, respectively, the difference was statistically significant(P<0.05).The operative time of partial resection in the two groups was 76.84±19.75 min and 90.13±20.07 min, respectively, and the difference was not statistically significant(P > 0.05).The operative time of total resection in the two groups was 87.53±25.18 min and 90.89±16.41 min, respectively, and the difference was not statistically significant(P > 0.05).The number of peritoneal injury cases in the two groups was 2 and 4,respectively,and the difference was not statistically significant(P > 0.05).The median and quartile range of total drainage volume were 162.5 m L and 196.0 m L in group A,respectively,121.5 m L and 86.75 m L in group B,there was no difference statistically significant(P>0.05).The median and quartile range of postoperative drainage tube carrying time in group A were 5.0d and 2.0d,respectively,while those in group B were 5.0d and 2.0d,the difference was not statistically significant(P>0.05).The median and quartile range of postoperative hospital stay were 6.0d and 3.0d in group A,respectively,6.0d and 3.0d in group B,the difference was not statistically significant(P>0.05).The median and quartile range of postoperative intestinal function recovery time of group A were 1.0d and 1.0d,respectively,and that of group B were 1.0d and 1.0d the difference was not statistically significant(P>0.05).Two groups of postoperative complications class were Ⅱ.Conclusion:1.Retroperitoneal laparoscopic single-layer method and traditional three-layer method can be well applied to adrenalectomy, and the surgical treatment effect is similar.2.The single-layer method has simple steps and shorter operation time. There is no significant difference in postoperative drainage and postoperative drainage tube carrying time between the two methods... |
