Objective: To analyze the current situation of carotid atherosclerosis and the level of associated risk factors in high-risk population of stroke in rural areas of Chaoyang, and to provide supporting data for comprehensive and effective evaluation of the burden of cardiovascular disease in high-risk population of stroke in rural areas of Liaoning province, and to provide theoretical basis for the formulation of related prevention and treatment strategies and targeted interventions. Methods: This cross-sectional study was conducted from September 2017 to May 2018.Permanent residents aged≥40 from 13 villages in Chaoyang and Lingyuan counties of Liaoning province were selected. All subjects were given questionnaires, laboratory tests, physical examinations, and carotid ultrasound. Then, stroke risk assessment was conducted on the subjects according to the "8+2" factors formulated by the National Health and Prevention Commission, and a total of 2,479 people with high risk of stroke were selected. Finally, the ultrasonic, biochemical indexes and questionnaire results of the target population were analyzed. Results: In rural areas of Chaoyang, there were 1062 males(42.8%)and 1417 females(57.2%)aged ≥40 years at high risk of stroke. The rates of CA,plaque presence, and CIMT thickening in high-risk population were 48.9%,46.5%,and 13.4%,respectively.In addition, the rates of CA,plaque presence, and CIMT thickening increased with age,the difference was statistically significant(P<0.01).The maximum plaque thickness/height increased with age in male, female and total population, and the difference was statistically significant(F=23.269,P<0.01;F = 18.456,P < 0.01;F=35.567,P<0.01).The patients at high risk of stroke had more combined risk factors, among which hypertension(84.4%),overweight/obesity(77.8%),and family history of stroke(51.6%)were the most significant.Multivariate logistic regression analysis showed that male, advanced age, high family income, hypertension, diabetes, current smoking, lack of exercise were the risk factors for CA in the high-risk population of stroke;senior age, high family income, current smoking and lack of exercise were the risk factors of CIMT thickening in high-risk population;Male, advanced age and diabetes mellitus are risk factors for plaque presence in high-risk groups. Conclusion: In rural areas of Chaoyang, the prevalence of atherosclerosis and related risk factors in high-risk stroke population aged 40 years and above is high and increases with age. Therefore, comprehensive management of atherosclerosis in high-risk population, especially elderly people, should be paid attention to reduce the disease burden.
