Objectives To analyze the changes of serum Neutrophil gelatinase-associated lipocalin(NGAL),Serum ferritin(SF)and Vitamin D(VD)levels in patients with Gestational diabetes mellitus(GDM)in the mid-pregnancy,and to explore the relationship between serum NGAL,SF and VD levels and the risk of GDM.Methods According to the diagnostic criteria of GDM recommended by IADPSG,pregnant women who underwent Oral glucose tolerance test(OGTT)in the outpatient department of Obstetrics,Hebei general Hospital during 24-28 weeks of pregnancy from July 2019 to October 2019 were selected according to the nested case-control study method.According to the OGTT results will be the research object is divided into two groups: namely the GDM group(65 cases)and Normal glucose tolerance(NGT)group(105 cases).The general clinical data of pregnant women were collected,and the peripheral blood of pregnant women at 24-28 weeks of gestation was collected.Serum NGAL,SF,VD levels were determined by Enzyme-linked immunosorbent assay(ELISA),and their newborn birth weight was tracked at the end of gestation.Results 1 NGAL,SF,TG(Triglycerides)and neonatal birth weight in the midpregnancy of GDM group were significantly higher than those in the Normal glucose tolerance group,and the differences were statistically significant(P<0.05).The midpregnancy Vitamin D(VD)and HDL-C(High density lipoprotein cholesterol)in GDM group was significantly lower than that in normal glucose tolerance group,and the difference was statistically significant(P<0.05).2 Correlation analysis of NGAL,SF,VD and blood glucose,blood lipid and other results showed that serum NGAL in the midpregnancy women was significantly positively correlated with SF,FPG(Fasting plasma glucose),FINS(Fasting insulin),HOMA-IR(Homeostasis model assessment insulin resistance),Hb(Hemoglobin),TG and neonatal birth weight,while NGAL was significantly negatively correlated with VD,HDL-C(P<0.05).SF level was significantly negatively correlated with VD and positively correlated with NGAL,FPG and Hb(P<0.05).VD was negatively correlated with NGAL,SF and FPG(P<0.05).3 Logistic regression analysis showed that NGAL,TG and SF were the risk factors of GDM,while VD and HDL-C were the protective factors of GDM(P<0.05).After adjusting for ferritin,Vitamin D and related blood glucose and lipid indexes,NGAL was significantly positively correlated with the risk of GDM and was an independent risk factor.Conclusions 1 NGAL is an independent risk factor for GDM in the mid-pregnancy,and NGAL is a marker of glucose and lipid metabolism disorder and islet β-cell dysfunction.Elevated plasma NGAL level indicates the deterioration of glucose and lipid metabolism disorder in patients with GDM.2 SF is a risk factor for the occurrence of GDM in the mid-pregnancy,so iron supplementation during pregnancy should not be excessive to avoid increasing the risk of GDM.3 VD is a protective factor for the occurrence of GDM in the mid-pregnancy,and low level of VD in pregnancy is associated with the occurrence of GDM.Figure: 3;Table: 4;Reference: 155...
