| Objectives To observe and clarify the effect of distal and local acupuncture in different order on the therapeutic effect of blood stasis type lumbar disc herniation(LDH)through a randomized comparative trial.Methods 142 patients with blood stasis type lumbar disc herniation were randomly divided into two groups to observe the effect of different sequence of acupuncture on the treatment of blood stasis type lumbar disc herniation.Both groups were acupunctured with the same acupoint.In group A,the distal acupoints were acupunctured,and then the local acupoints were acupunctured.Group B was in reverse sequences.In the process of acupuncture,after arrival of qi,moderate and equi-librious acupuncture technique was used for 1min,and then the next acupoint was acupunctured.The needle was kept for 30 minutes each time,and the treatment was done once a day,6 times as a course of treatment.After 1 course of treatment,rest for 1 day,and treatment for 3 courses of treatment.The efficacy was evaluated by VAS and JOA scales at the appropriate time.Results 1 Two patients fell out,and eventually 70 patients were included in both groups.2 After the treatment,the total effective rates of A and B groups were 88.57% and78.57%,respectively,and There was significant difference between the two groups(P <0.05).3 After treatment,JOA scores were significantly increased and VAS scores were significantly decreased in both groups compared with before treatment(P < 0.05).After treatment,JOA score in group A was significantly higher than that in group B,and VAS score was significantly lower than that in group B(P < 0.05).4 JOA and VAS scores of patients in group A and B were significantly increased and significantly decreased during follow-up compared with those before treatment(P < 0.05).The efficacy of both groups could be maintained within A certain period,and the maintenance effect of group A was better than that of group B(P < 0.05).5 Comparison of patients with different disease grades before and after treatment showed that the total effective rate of patients with JOA score of "acceptable" grade was 87.04% in group A and 78.57% in group B,the results were statistically significant(P < 0.05).There was no significant difference in the effective rate between "good" grade patients(P > 0.05).conclusions 1 Both the sequence of distal to local acupuncture and the sequence of local to distal acupuncture have significant curative effect on patients with blood stasis type lumbar disc herniation.2 Therapeutic effect of distal to local acupuncture sequence in treating patients with blood stasis type lumbar disc herniation is better than that of local to distal acupuncture sequence.3 The therapeutic effect of the two groups in the treatment of blood stasis type lumbar disc herniation patients can be maintained in a certain period,and the maintenance effect of group A is better than that of group B.4 The JOA grade of "acceptable" had a better therapeutic effect on the condition of patients with the acupuncture sequence of distal to local,while the "good" grade had no significant therapeutic effect difference.Figure 1;Table 17;Reference 99... |
