| Evaluation of Stability of Bone Subtitute MaterialPost-Lateral Sinus Floor Elevation Based on CBCTObjective To observe the changes of the vertical height of bone substitute material after maxillary sinus floor elevation by CBCT,and to study the influence of various related factors on the stability of bone substitute material,so as to provide clinical reference for the design of preoperative treatment plan and expected treatment effect of maxillary sinus floor elevation. Methods From May 2015 to March 2021,18 patients who were processed in theDepartment of Dental Implant,Hangzhou Stomatological Hospital, underwent maxillary sinus elevation and delayed implantation with simple bone substitute materials, and had completeCBCT imaging data before, immediately after and 9 months after operation, wereretrospectively studied. The changes of bone substitute materials such as residual bone height(RBH)before operation, bone depth after operation and bone height 9 months after operation were observed, and the influence of patients’gender, age, number of missing teeth, missing tooth site, residual alveolar bone height, maxillary sinus angle, maxillary sinus width, processing mode of bone substitute materials and vertical bone increment height on the stability of bone substitute materials in sinus after maxillary sinus floor elevation was analyzed.Results1.The average preoperative residual bone height was(3.66±1.86)mm;the immediate average bone height after operation was(16.61士2.85)mxn;The mean bone height was(15.47±2.71)mm at 9 months after operation;the average increment bone height afteroperation was(12.82±3.37)mm;The average sinus angle was(79.35.±18.72)~°;The averagesinus width was(13.77士2.07)mm;The mean bone height reduction at 9 months afteroperation was(1.14±〇.85)mm;The average bone height reduction rate at 9 months after operation was(9.24±7.13)%.2.There is a significant difference between preoperative and postoperative bone depth(p~=0.000<0.05);There is significant difference in bone height between preoperative and postoperative 9 months(/?~=0.000<0.05).There is no significant difference in bone height between immediately after operation and 9 months after operation(p~=0.251>0.05).3.There was no significant difference in bone height reduction rate among different ages, genders, missing tooA sites, residual bone height, maxillary sinus width and maxillary sinus angle. That is,there is a positive correlation between the increase of bone depth and the decrease of bone height after operation. After 9 months of operation, the bone height reduction rate of bone substitute treated with local blood was lower than that of bone substitute treated with normal saline, and there was a statistical difference between them. Conclusion 1.Bio-Oss bone substitute has good stability and low bone height reduction at 9months after operation,so it is recommended to be used in maxillary sinus elevation.2.Gender,age?missing tooth site, residual bone height?width of maxillary sinus and angle ofmaxillary sinus have no influence on bone height reduction and bone height reduction rate of bone substitute materials.3.Smaller incremental height of bone after operation are beneficial to reduce the bone height reduction of bone substitute material and improve the stability after lateral maxillary sinus elevation.4.It is recommended to process bone substitute materials with local blood instead of normal saline to reduce the resorption rate of bone substitute materials after operation and improvethe stability. |
