| Objective: To explore the core drugs and drug characteristics of Professor Ba Yuanming in the treatment of DKD by data mining of the characteristics of the drug use and the law of treatment of DKD patients treated by the tutor Professor Ba Yuanming in outpatient service,combined with the network pharmacology analysis method.Professor Ba Yuanming has been engaged in medical clinical work for more than 30 years and has rich experience in clinical chronic kidney disease.This study combines data mining and network pharmacology research,which can not only explore Professor Ba Yuanming’s academic experience in treating DKD,but also It can predict the mechanism of action of core drugs in the treatment of DKD,and contribute to the inheritance of famous and old Chinese medicine.Methods: Part 1: By collecting the medical records of DKD patients diagnosed and treated by Professor Ba Yuanming of Hubei Provincial Hospital of Traditional Chinese Medicine from June 2020 to June 2021,the general information,symptoms,tongue diagnosis,Chinese and Western medicine diagnosis and Chinese medicine prescription of the patients were collected according to Organize in a unified format.80 cases that met the requirements were screened,and a database was established on the ancient and modern medical record cloud platform.The rules were summarized by statistical methods such as frequency analysis,association rule analysis,and cluster analysis,and the core drugs and prescription rules of Professor Ba Yuanming in the treatment of DKD were discussed.The second part uses the TCMSP database to collect the chemical components of the core traditional Chinese medicines,obtains the relevant targets of the drugs through the SIB database,and uses the Uniprot database to normalize the target genes;DKD disease-related genes are searched in the Genecard database,and the disease genes are associated with drug targets.The common potential gene targets were screened out after combining and deleting duplicates;the PPI network relationship was constructed using the STRING database,and the PPI network relationship was topologically analyzed by the "Network Analyzer" function of Cytoscape software to screen out key targets;the David database and R language for GO and KEGG enrichment analysis and visualization to predict the possible mechanism of action of core drugs in the treatment of DKD.Results: The first part: Among the 80 patients collected and included,the ratio of male to female was about 1.35:1,and there were more males than females,and most of them were in the age range of 50-69 years old.The research involves 98 traditional Chinese medicines,with a cumulative frequency of1337 times.The 10 high-frequency drugs are: Astragalus,Chinese yam,Rehmannia glutinosa,Codonopsis pilosula,cooked rhubarb,dogwood,Cork,Poria,madder,and Rhizoma Imperatae.The top five are the core drugs;The most used medicines are heat-clearing and blood-cooling medicines,water-dampening medicines,swelling-reducing medicines,stagnant-blocking medicines,and fluid-boosting medicines.the meridians entered are the Kidney Meridian of Foot Shaoyin,Lung Meridian of Hand Taiyin,Liver Meridian of Foot Jueyin,Spleen Meridian of Foot Taiyin,Stomach Meridian of Foot Yangming;the common clinical symptoms of patients are frequent nocturia,fatigue,low back pain,edema and sleep.Poor;the common clinical syndromes are Yin deficiency and Fire Prosperity syndrome,Qi and Yin deficiency syndrome,and internal dampness syndrome.Part II:Matching core drugs with DKD diseases to obtain 218 potential targets,involving a total of 74 compounds,and finally obtaining 33 core targets through PPI network analysis,such as: STAT3,SRC,MAPK1,IL6,PIK3 CA,etc.The results of GO enrichment and KEGG enrichment showed that: PI3K-Akt signaling pathway,AGE-RAGE signaling pathway and RAS signaling pathway were mainly involved.Conclusion: Through the results of data mining,it is found that when Professor Ba Yuanming treats DKD,he mainly focuses on nourishing qi and nourishing yin,clearing heat and removing dampness,promoting blood circulation and removing blood stasis.The core drugs consist of astragalus,yam,rehmannia glutinosa,Codonopsis pilosula,and cooked rhubarb,which may be involved in the regulation of cell proliferation and apoptosis,protection of podocytes,anti-inflammatory effects,and anti-renal fibrosis to interfere with the progression of DKD. |
