| Objective:To observe the function of Bushen Yuyin Huoxue Recipe on the DOR in patients with OEC after laparoscopic surgery,objective to evaluate the effect of Bushen Yuyin Huoxue Recipe on DOR after laparoscopic surgery for Ovarian endomtrial cyst,and to provide a new method and idea for the clinical treatment of this disease.Methods:A total of 45 patients with decreased ovarian reserve after laparoscopic surgery for Ovarian endomtrial cyst,the clinical trial was conducted by using the single-group before-and-after control test method.The patients were treated withKidney nourishing Yin activating blood formula,1 dose daily,take it in the morning and 2 times in the afternoon when warm,1 month for a course of treatment,6 months for a total of 6 courses.The patients were observed for clinical comprehensive efficacy after the end of treatment.Records before and after treatment 1 month,3 months,6 months period of score,score of syndromes and treatment of patients before and after serum AMH,FSH,FSH/LH and the value of an AFC,to assess the effectiveness of traditional Chinese medicine treatment of the disease.Result:1.Menstruation score and TCM syndrome score analysis:The TCM syndrome score and menstruation score of kidney deficiency and blood stasis type OEC before and after treatment were significantly decreased(P<0.01).The improvement after treatment varied with different age stages and duration of disease.Comparison within age groups:the scores of patients in≤28 years old group and 29-34 years old group after treatment were significantly different(P<0.01);≥ 35 years old group patients after treatment scores difference compared with before treatment(P<0.05).Comparison among groups:≤28 years old group and 29-34 years old group score was no significant difference(P>0.05);There were statistically significant differences(P<0.05)in scores between the≤28 years old group and the ≥35 years old group,and between the 29-34 years old group and the ≥35 years old group.Intra-group comparison of course of disease:post-treatment scores of patients in the three age groups were significantly different(P<0.05)from those before treatment.Comparison between groups:<6 month group and 6-12 month group scores are not significantly different(P>0.05);<6 month group and>12 month group,6-12 month group and>12 month group scores the difference(P<0.05)was statistically significant.Statistical analysis showed that traditional Chinese medicine can improve the ovarian function of DOR patients,age was correlated with menstruation and improvement of TCM syndromes.The younger the age,the better the improvement of menstruation and TCM syndromes,the effect of TCM treatment on menstruation and TCM syndromes was better in 1 year and a half after operation.2.Comparison of serum AMH levels:After treatment,AMH of patients was significantly increased compared with that before treatment,P<0.01,was statistically significant.Improvement after treatment at different ages with different courses of disease has differential.Comparison within age groups:≤ 28 years old group or 29-34 years old group patients after treatment of AMH level to improve before the treatment(P<0.05),statistically difference,and the ≥35 years old group was treated the level of AMH after treatment was not significantly different(P>0.05)from that before treatment;Between group comparisons:it was not meaningful(P>0.05)in AMH levels after treatment between the≤28 years old group and the 29-34 years old group,after treatment,the AMH level in the ≤28 years old group、the 29-34 years old group and the≥35 years old group had statistical significance(P<0.05).Within disease duration group comparisons:patients in the<6-month group and the 6-12-month group had statistically significant(P<0.05)higher post-treatment AMH levels than those in the pre-treatment group,AMH level changes after treatment in patients with>12 months age groups(P>0.05),no statistical difference;Among groups:AMH in the<6 month group compared to the>12 month group(P<0.05),the<6 month group and the 6-12 month group,the 6-12 month group and the>12 month group there was no statistically significant difference(P>0.05)in AMH comparison.Statistical analysis showed that there was correlation between age and AMH improvement.Patients with DOR of Ovarian endomtrial cyst postoperative should be treated as soon as possible to improve the level of AMH,especially before the age of 28,Chinese medicine treatment within 1 year after surgery has a better effect on AMH improvement.3.Comparison of serum FSH,FSH/LH levels:After treatment,the serum FSH,FSH/LH levels of patients were improved(P<0.05).Improvement after treatment at different ages with different courses of disease has differential.Within age groups,the levels of FSH and FSH/LH in the≤28 years old group and the 29-34 years old group were significantly lower after treatment than before(P<0.01).The levels of FSH and FSH/LH in the≥35 years old group were not significantly decreased after treatment(P>0.05).Comparison between groups:three age groups after treatment of FSH,FSH/LH improvement no statistical difference(P>0.05).Within disease duration group comparisons:patients in the<6 month group,the 6-12 month group had lower post-treatment FSH and FSH/LH levels than those in the pre-treatment group(P<0.05),while patients in the>12 month group had no post-treatment FSH and FSH/LH levels(P>0.05);it has no significant difference(P>0.05)in the decrease of FSH and FSH/LH among the three disease course groups after treatment.The statistical analysis showed that the patients before 35 years old were given traditional Chinese medicine within 1 year and a half after surgery,especially from postoperative to 1 year after surgery The therapeutic effect is better.4.Comparison of AFC level:the AFC level of patients after treatment was significantly increased compared with that before treatment(P<0.01).Improvement after treatment at different ages with different courses of disease has differential.Within age group comparisons:The level of AFC in the≤28 years old group and the 29-34 years old group after treatment was significantly different from that before treatment(P<0.05),while the level of AFC in the≥35 years old group after treatment was not significantly different from that before treatment(P>0.05);comparison among groups:≤28 years old group or 29-34years old group level of AFC was no significant difference(P>0.05);There were significant differences(P<0.05)in AFC levels between the ≤28 years old group and the≥35 years old group,and between the 29-34 years old group and ≥35 years old group.Within disease duration group comparisons:patients in the<6-month group and the 6-12-month group had statistically significant(P<0.05)higher post-treatment AFC levels compared to pre-treatment values,>the 12-monthgroupThe AFC level of patients was not statistically different(P>0.05)after treatment;comparison between groups:it was no statistical significance(P>0.05)in AFC increase among the three disease duration groups after treatment.Statistical analysis showed that DOR patients within 1 year and a half after the operation and before the age of 35 years old had better treatment effect,especially the conditioning effect before the age of 28 years old.Conclusion:According to statistical analysis,Bushen Yuyin Huoxue Decoction can improve TCM syndromes and menstrual irregularities in patients with DOR after Ovarian endomtrial cyst operation,and can effectively improve AMH levels,sex hormone levels and basic antral follicle levels in patients.After treatment the total effective rate of this study was 85.00%,which showed that the curative effect of traditional Chinese medicine in the treatment of DOR patients after Ovarian endomtrial cyst surgery was good,which could improve the ovarian reserve function of patients,and there were no adverse reactions and adverse events in the treatment,and it was safe and reliable,which was worthy of further promotion in clinical practice. |
