ObjectiveOptimization of the new nano experimental conditions of circulating tumor cell separation microfluidic chip,detection of circulating tumor cells;the positive rate of detection of circulating tumor cells in pancreatic cancer Cell Search,evaluate the feasibility of detection of circulating tumor cells in pancreatic cancer;comparison of pancreatic cancer patients,the number of patients with benign pancreatic lesions and healthy volunteers in circulating tumor cells to investigate the clinical significance of detection of circulating tumor cells in diagnosis and prognosis of pancreatic cancer.Methods7.5ml venous blood collected from healthy volunteers peripheral,the tumor cell line GFP transfected with simulated circulating tumor cells,respectively in 1.5ml/min,2.0ml/min,1.0ml/min velocity,2.5ml/min,3.0ml/min of the sample,calculated the nano micro fluidic chip capture efficiency,obtain the optimal sampling speed;simultaneous acquisition of pancreatic cancer patients from the peripheral vein of morning blood 7.5ml examined Cell Search and nanometer microfluidic chip enrichment of pancreatic cancer circulating tumor cells by immunofluorescence(Ep CAM,CK-18,CD-45,DAPI)identifycation,compare in the pancreatic cancer CTCs capture the effect of acquisition;patients with benign pancreatic lesions and healthy volunteers in the morning 7.5ml in peripheral blood,using nano detection of circulating tumor cells microfluidic chip,combined with clinical treatment,the correlation analysis for detection of circulating tumor cells and pancreatic cancer diagnosis and prognosis.ResultsIn 1.0ml/min,1.5ml/min,2.0ml/min injection speed of nano micro fluidic chip average enrichment rate for 57.7%,66.3%,75.2%;2.5ml/min and 3.0ml/min for the sampling process repeated separation system blocked,repeatedly restart to eliminate the effect of fluid stability;comprehensive nano micro fluidic chip capture efficiency and patency,2.0ml/min was chosen as the best injection speed.From October 2015 to March2017,a total of 36 patients with pancreatic cancer were collected from the study,and a total of 10 patients with benign pancreatic diseases and healthy volunteers(n=10)were enrolled in this study.Among them,20 patients with pancreatic cancer were examined by Cell Search,and the positive rates of Cellsearch and nano micro fluidic chip in the detection of pancreatic cancer were CTCs and 100%,respectively.There was significant difference between the two groups(P=0.0001).Conclusion and significance:The characteristics of nano microfluidic chip with high throughput,high enrichment separation in CTCs capture rate,the sample rate is 2ml/min when the rate of concentration is high;compared to Cell Search,nano microfluidic chips have a higher positive rate in the detection of pancreatic cancer CTCs;the sensitivity of nano micro fluidic chip of pancreas CTCs cancer detection was 96.6%,specificity was 86.7%,CTCs detection can be used as an important supplement to clinical pancreatic cancer diagnosis;clinical analysis of pancreatic cancer CTCs count and tumor metastasis correlated,CTCs can be used as an important indicator to predict the detection of pancreatic cancer metastasis.
